This position is primarily focused on coordinating the referral process to support access to care for individuals and families served by the organization. We offer:

Duties include and are not limited to:
+ Monitor national points of access to care and respond to all incoming activity – inquiries and referral leads
+ Ensure documentation of all inquiry/referral interactions, including referral entry and referral/admissions tracking reports on an ongoing basis.
+ Referral coordination – initiate referral entry, review, distribution for center review, provide support navigating the Devereux continuum of care
+ Internal/external support for centers with referral response, data entry and initiation of the referral review process
+ Coordinate implementation and maintenance of data systems managing referral/admissions records, such as the EHR (MyAvatar) and CRM functions, network directories and customer databases.
+ Customer relations support for referral sources to access care at Devereux
+ Develop inventory of conferences and sponsorship opportunities
+ Develop guidelines for all levels of staff to represent Devereux at events, such as exhibit booths, information sessions, etc.
+ Maintain organization profile and support access to care in published materials and directories
+ Documents referrals timely and monitor systems for data integrity
Qualifications
Qualifications
+ Bachelors degree, preferred in human services field.
+ Minimum 1-2 years of healthcare, social services, or related experience.
+ Experience interacting in a healthcare or social services environment, experience with referral coordination, medical record and data management, effectively communicating in verbal and written format with a customer-friendly approach, maintenance of data collection and analysis and implementing systematic process to ensure fluid workflow and responsive support.
+ Experienceinteracting withpeople from diverse backgroundsandacompassionate understanding ofinformation shared via thereferral process, to include understandingof best practices related to privacy, confidentiality, trauma informed care, etc.
+ Electronic medical record experience and/or proficiency with technological tools such as MSOffice, Word, Excel, etc. desired.
+ Excellent writing skills are required, especially the ability to tailor messaging to varying audiences, such as parent/guardian, funder/agency partner, and education and healthcare professionals Experience with creating compelling systems for data management. Strong verbal, presentation and proof-reading skills.
+ Ability to handle multiple projects and tasks from a variety of staff and demonstrate initiative in a fast-paced environment.
