    What You’ll Do:

    Collaborate with medical providers to ensure whole-child, family-centered care

    Help families overcome barriers to care by addressing social determinants of health

    Serve as a trusted partner and resource to caregivers, connecting them with vital services and support

    Required Education:

    Master’s Degree in Social Work (MSW)

    Skills & Experience:

    Strong problem-solving, critical thinking, and communication skills

     

    Ability to work independently with attention to detail and high computer literacy

     

    Proven ability to build rapport with patients, families, and interdisciplinary teams

    Licensure:

    Active LISW license
