    BayCare HomeCare is currently in search of our newest **Home Care Scheduler (Team Assistant)** who is passionate about providing outstanding customer service to our community. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.

     

    + **Facility:** Sarasota Memorial HomeCare

    + **Location:** Sarasota, FL

    + **Status:** **PRN (As needed)**

    + **Shift Hours:** 8:00 AM - 5:00 PM (Weekdays Varies)

    + **Weekend Work:** Occasional

    + **On Call:** No

    Home Care Scheduler Responsibilities:

    + **The Team Assistant is responsible for scheduling and oversight for all disciplines:** Nursing, Therapy, and Social Work

    + Assists in entering approved PTO and patient Emergency Management Plans into scheduling package

    + Enters and verifies vital patient information into billing system

    + Maintains CPR card, Driver's license and Auto Insurance information in team member files

    + Reviews admission packets for completeness

    + Assists the Home Care Supervisor in productivity oversight

    + Works all report as indicated in standard work

    + Adds and approves all disciplines as instructed by the Supervisor, including the visit type

    + Enters skills for each clinician in staff management area of scheduling package and enters assignment of and updates for case manager for each patient

    + Assists with daily routine paperwork and clerical duties necessary for the smooth day to day function of the unit

    Education:

    + Required High School or Equivalent

    + Preferred Associates Computer Related

    Experience:

    + Required 1 year Computer Related

    + **Preferred 1 year Home Care**
