• SUD Service Coordinator I

    UPMC (Pittsburgh, PA)


    Provides SUD support and counseling, according to the level of need, to a population of individuals with a serious mental health diagnosis and SUD. The primary function of SUD Service Coordination is to provide professional assessment, service planning, coordination of D/A supports, submission of referrals, and re-evaluation services required for a safe and healthy community life which is manifested through stability in relationships, housing, and employment. This is done in accordance with the Agency's vision, mission, and values, and emerging mental health Recovery Principles. This position provides an essential recovery orientation.

     

    This position is eligible for sign-on bonus of $3,000 for an 18-month work commitment!

    Responsibilities:

    Coordination and Advocacy

    + Coordinate D/A support and services based on individual needs.

    + Advocate for individuals using the "harm reduction" modality.

    + Develop relationships with individuals, their families, and key contacts.

    Assessment and Planning

    + Assess individual and family strengths and needs collaboratively.

    + Develop, document, and implement comprehensive service plans.

    + Complete assessments and documentation with individuals.

    Training and Compliance

    + Attend training programs to stay updated on best practices.

    + Maintain contact during hospitalizations and transitions of care.

    + Keep accurate records of SUD Service Coordination activities.

    + Update forms and review charts for regulatory compliance.

    + Bachelor's Degree in a Human Services field such as Social Work, Psychology or Counseling required.

     

    _Other fields of study may be considered with coursework of at least 12 credits in Human Services._

     

    + One-year related experience preferred.

    + Prior experience working in community settings with SUD population preferred.

    + Safe driving record required.

    Licensure, Certifications, and Clearances:

    + Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

    + Cardiopulmonary Resuscitation (CPR)

    + Comprehensive Crisis Management (CCMC)

    + Driver's License

    + Act 31 Child Abuse Reporting with renewal

    + Act 33 with renewal

    + Act 34 with renewal

    + Act 73 FBI Clearance with renewal

    + OAPSA
