    Job Summary

    The Supervisor Community Based is responsible for the delivery of quality services to youth, families and adults and the performance of the direct care staff in a community based program to meet the needs of clients. This role is responsible for the efficiency of the day to day operations of the program and the safety of youth, adults, families and staff.

     

    This is full-time (40 hours) position on primarily a Monday-Friday 8:30am-5:00pm schedule, though flexibility for some occasional evening/weekend hours is required.

     

    This team and role covers the Greater Erie/Buffalo Region, and travel to/around that region is required for the role, but there is flexibility for a hire to be based elsewhere within Hillside’s regions of coverage in Upstate, NY as long as they are able to commute to that region as needed.

     

    Essential Job Functions

     

    + Oversee effective and efficacy of the day to day operations of the program.

    + Maintain relationships and the intake and referral process from the various counties and agencies.

    + Ensure staff deliver comprehensive adult focused and or youth guided, trauma-informed, family driven services within the communities they serve.

    + Ensure all funder and agency documentation and reporting requirements are met by staff Ensure compliance with regulatory bodies.

    + Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators, performance measures, client satisfaction, and plan adherence.

    + Performs direct care staff functions as assigned and required.

    + May assume the responsibility of staff and/or program coverage as needed, for the Program Manager Community Based, in their absence.

    + While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.

    LEADERSHIP DUTIES

    + Leadership: Communicates supports and links Hillside mission, vision, values, goals, and strategies to everyday work and establishes clear, meaningful, challenging, and attainable objectives and expectations that are aligned with those of the organization.

    + Management: Aligns the right work with the right people; delegates tasks according to people’s strengths and interests. Ensures staff has the skills and resources to be successful. Provide staff with coaching, training and opportunities for growth to improve their skills. Treats staff fairly and consistently. Shares accountability when delegating and involves staff in setting their performance goals.

    + Governance: Ensures Hillside policies, procedures and all applicable governmental laws/regulations/guidelines are followed. Makes decisions and has authority to implement decisions in conjunction with the leadership structure of the given area of responsibility.

     

    Education & Experience

     

    + Bachelor’s degree in a related field required.

    + Minimum 2 years of relevant experience required.

    SPECIAL REQUIREMENTS

    + Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.

    + CPR and First Aid certification required or obtained within 60 days of hire.

    + Ability to monitor program performance, collect data, and evaluate outcomes to drive continuous improvement.

    + Knowledge and awareness of treatment models and approaches for the particular program.

    + Knowledge of and ability to ensure all funder, COA and agency related compliance standards are met by staff.

     

    Physical Demands & Work Environment

     

    The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

     

    + Must be able to work a variable scheduling including evenings and weekends

    + The following daily physical demands are required:

    + Sitting (up to 6 hours)

    + Occasional standing (up to 2 hour)

    + Occasional walking (up to 2.5 hours)

    + Driving (several times a week up to 6 hours)

    + Continuous balancing (up to 8 hours)

    + Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)

    + Weekly lifting up to 10-15 lbs. on a daily basis

    + Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)

    + Occasional exposure to dust, fumes, gases, chemicals, or smoke is apparent

    + Ability to change positions as needed

    SPECIAL CONSIDERATIONS

    While Hillside is a restraint free environment, there may be times in a “life or limb” situation, that staff may be required to physically restrain clients weighing between 50 lbs. and 300 lbs. and guide them safely to the floor. In some circumstances, restraints can last up to 15 minutes and may require repetition as necessary.

     

    $23.35 Minimum pay rate, $33.00 Maximum pay rate, based on experience. This pay rate may include a Regional and/or Department premium.
