On-the-job educational programs for nurses, psychologists, advanced practice providers, psychiatrists, and chaplains are equipping the next generation of care providers.

What Will you Do?
As a Care Navigator at Pine Rest, you’ll be part of tight-knit team that believes in the healing power of your expertise and compassion. You’ll work with the specialty clinic teams (NDD and PPUCC) at the Pediatric Center of Behavioral Health assisting caregivers, referring providers, and other community partners in finding and securing the best treatment options available within the Pine Rest continuum of care for pediatric patients. This position will communicate with families/caregivers, referring providers, satellite Pine Rest clinics, schools, Helen DeVos Children’s Hospital, and other partner agencies to assist in triaging patient referrals to ensure proper and appropriate placement. They will demonstrate exemplary interpersonal skills, professionalism, and knowledge of Pine Rest services, spanning Urgent Care, Inpatient, Partial Hospitalization, and Outpatient programming. They must demonstrate awareness of existing services available in the community and have the competency to make recommendations for a step-up or step-down in patient treatment. This position will require excellent time management and communication skills.

Principal Duties and Responsibilities:
+ Review patient referrals for completeness and eligibility in Pine Rest programming, including monitoring census and verifying insurance.
+ Connect with referring providers for additional documentation or information as needed.
+ Demonstrate proficiency in existing Pine Rest services (including specialty clinic exclusion and inclusion criteria) throughout the continuum of care and refer patients as appropriate.
+ Schedule admissions and any follow-up care for patients.
+ Communicate with schools about discharge dispositions and patient needs or access requirements.
+ Patient Advocacy: Advocate for patients and their families, ensuring their needs are met and that they receive the best care possible.
+ Care Coordination: Coordinate prospective future patients, current patients, and discharged patients’ appointments, referrals, and follow-up communications to ensure continuity of care.
+ Resource Navigation: Connect patients and their families with relevant community resources, support services, and financial assistance programs. Assist referring providers in identifying the appropriate treatment for their patients within the Pine Rest system.
+ Education and Support: Educate patients, their families, and other community partners about available programs, treatment options, and the Pine Rest system, empowering them to make informed decisions.
+ Communication and Liaison: Facilitate communication between patients, the families, and healthcare providers, ensuring everyone is on the same page.
+ Problem Solving: Help patients and their families overcome barriers to care, such as transportation, language barriers, and other financial concerns.
+ Monitoring and Evaluation: Monitor patient program and outcomes, adjusting navigation strategies as needed.
What Does the Role Require?
Education/Experience:
+ Bachelor’s degree in psychology, social work, or other areas related to Mental or Physical Health.
+ A minimum of one year of experience in a patient-facing role.
