Summary This Clinical Pharmacist will provide a broad range of professional and clinical pharmacy services in a geographically vast, highly rural medical center. The incumbent will be responsible for the pharmacy care in the Community Based Outpatient Clinics (CBOCs) including but not limited to Community Care, the Non-VA Care, and the Veterans Choice programs. Ensures all prescriptions coming from Non-VA providers are processed, working with the providers, patient, and other services to ensure timely delivery of these services within Pharmacy Service Policy. Evaluates medication orders and medication profiles for appropriateness of therapy, dosage range, synergistic, antagonistic, and cumulative effects. Provides consultation and education to physician, nurses, and other health care professionals on drug information. Evaluates provider's orders for appropriateness based on patient medication profiles; inputs information into computer system; dispenses medication in accordance with appropriate formulary requirements. Works with Pharmacy Service procurement employees to ensure adequate stock and avoidance of out of stock situations. Prioritizes work to meet operational and patient care needs. Ensures the appropriate utilization, maintenance, and care of Pharmacy Service resources. Assists customers at the pharmacy window and answers the telephone. Monitors and analyzes orders and distribution patterns to identify questionable circumstances or unusual trends that may suggest diversion or increased usage. Creates an environment to stimulate, encourage and support the participation of pharmacy personnel in providing drug information to the hospital staff and other facilities as deemed appropriate. Attends and actively participates in pharmacy activities, i.e., staff development, pharmacy meetings, etc. Provides a framework for continual learning for pharmacy students, residents, and employees of all levels to ensure competence and improve performance. English Language Proficiency: Pharmacists must be proficient in spoken and written English. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12 Clinical Pharmacist (Full Performance Level) Experience or Education. In addition to the basic requirements, candidates must meet one of the following: 1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.D. program. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Physical Requirements: There may be some walking, standing, bending, sitting, and carrying of light items. Incumbent must be physically and mentally able to perform efficiently the duties of the position. The work requires regular and recurring physical exertion, involving standing for prolonged periods of time, lifting/carrying of pharmaceutical supplies weighing at least 50 lbs., and bending/stooping.
