    Summary The GS-12 Social Worker Complex Care/Guardianship Program Coordinator is administratively responsible for management and coordination of Complex Care and Guardianship cases and processes within the Inpatient setting and Social Work Service. They provide direct clinical services with program coordination responsibilities. They must be licensed at the advanced practice level and must be able to provide clinical supervision for subordinate staff to qualify for licensure. Responsibilities Duties include, but are not limited to: Oversees the daily operation of care delivery, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use Provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service May be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds Administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use Provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for staff Initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports. Oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts Develops, implements, evaluates and reports on program performance, including assigned measures, monitors, data and other metrics of interest Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Inputs data into required program databases as appropriate Develops monitoring tools to track program performance. Submits action plans to address areas requiring improvement Utilizes recognized process improvement tools to identify areas needing improvement and strategies for improvement. Incumbent is able to demonstrate the ability to design changes based on empirical findings. The incumbent provides timely and accurate information both verbally and in writing for reports to the Chief of Social Work Service, and others as indicated by program requirements Ensures that social work practice in assigned settings is evidence-based and in keeping with recognized social work treatment methods Explains veteran's treatment and progress to third parties as appropriate. Manages the quality control and evaluation of the professional services provided. Devises methods of obtaining data and evaluates data to determine the effectiveness of treatment. Identifies consistent qualities of successful treatment and of failed treatment Ensures that clinical care and processes within the assigned program meets the standards prescribe by local codes and is within the deliver care standards and guidelines in accordance with applicable grant and contract proposals, as appropriate Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested Maintains and manages an inpatient caseload Provides direct care services to Veterans in the inpatient setting to include assessment, risk screening, appropriate treatment interventions and discharge planning Documents social work interventions and activities in patient's clinical record and ensure appropriate hand off when transferring of patient care Maintain a current, unrestricted driver's license Acts as a liaison between VA and community resources, to include marketing and public relations duties as needed Work Schedule: Monday - Friday, 7:30am - 4:00pm Telework: Not Available Virtual: This is not a virtual position. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Language: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a Master's Degree in Social Work from a school of Social Work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of Social Work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A Doctoral Degree in Social Work may not be substituted for the Master's Degree in Social Work. Verification of the degree can be reviewed in CSWE website to verify the Social Work degree meets the accreditation standards for a Masters of Social Work. Licensure. Must be licensed or certified by a state to independently practice Social Work at the Master's Degree level. Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring Knowledge and ability to write policies, procedures, and/or practice guidelines for the program Ability to supervise multidisciplinary staff assigned to the program Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s) Ability to provide training, orientation, and guidance within clinical practice References: VA Handbook 5005/120, Part II, Appendix G39 Physical Requirements: Traveling throughout the medical center and the community is required, as is performing activities involving sitting, walking, and standing, bending and carrying such items as books, papers, and files. In carrying out responsibilities it may be necessary for the incumbent to travel into the community where he/she conducts interviews with the Veterans, their families, representatives of the community. The incumbent must possess current driver's license and drive a government vehicle in carrying out professional duties when deemed necessary.
