Description:
Responsible for the home evaluation and treatment of clients with physical and/or emotional limitations. Educates patients and families to maximize patient function and independence through physical therapy. Adheres to MercyOne's performance expectations and performs all defined services and other related duties in accordance with the mission, vision and values of MercyOne.

ESSENTIAL FUNCTIONS
+ Evaluates patients to ensure appropriate treatment.
+ Designs and implements plan of care.
+ Provides education to patients and families.
+ Adheres to home health guidelines.
+ Maintains a medical record for each patient.
MINIMUM QUALIFICATIONS
+ Master’s Degree in Physical Therapy, PhD preferred.
+ Home care experience preferred.
+ Licensed Physical Therapist in the state of Iowa.
+ Mandatory Reporter BLS.
+ Valid IA driver’s license and an acceptable Motor Vehicle Report (MVR). Vehicle must have current insurance.
+ High language, intermediate math, high reasoning ability.
+ Personal computer.
+ Ability to work with a large degree of independence, has reliable transportation to get to assigned cases, good organizational skills, willingness to work in adverse conditions.
