Position Summary:
Florida Atlantic University is seeking a Community Healthcare Worker - Boca Raton, FL.
Summary of Responsibilities:
Under the guidance of the project manager and community social worker, the Community Healthcare Worker (CHW) will collaborate with the entire project team in the REACH Community evidence-based intervention program.
+ Develop eligibility criteria guided by the REACH Community evidence-informed intervention.
+ Conduct training programs tailored to MWC-Davie staff, including all information on outreach, eligibility, referrals, and communication.
+ Works closely with the Center Director, the grant project manager, and each member of the team/staff at the Clinic to meet the complex needs of persons serving and exemplifies the mission of the Memory and Wellness Center and the philosophy of the College of Nursing and guides students’ practicum experiences as needed.
+ Recruit family/friend caregivers for REACH Community through ongoing internal identification protocols (home visits, support groups, community partners) encountered directly or through networking and referral processes.
+ Serve as liaison between Community Partners and the grant project team.
+ Liaison with Community Partners in identifying and referring individuals with ADRD who live alone, as well as the needs of individuals and families, and offer recommendations on resources and services to meet everyone and family caregiver’s unique needs through the grant activities.
Implement coaching sessions for caregivers of individuals with Alzheimer’s disease and related dementias (ADRD) based on the care plan results.
+ Complete a REACH risk assessment with each caregiver of an individual with or at risk for ADRD.
+ Coordinate and provide support for family caregiver needs assessment, behavioral symptoms management training, consultation, and care planning.
Work with the social worker(s) on scheduling follow-up visits, in-person visits and check-in calls with a list of services to be provided to persons living alone, which are directly arranged and coordinated by the CHW.
+ Plan and coordinate with the social worker(s) to implement in-person care consultation and develop an individualized care plan (and revisions).
+ Conduct follow-ups monthly and weekly check-ins with individuals living alone with ADRD.
+ Determine new services and supports needed for individuals with ADRD living alone.
+ Lead new support groups with those individuals newly diagnosed with ADRD.
+ Operate as part of an interprofessional team with Community Partners, individuals living with ADRD, and family caregivers.
Assist in event planning and organization for the programs and activities, including educational activities, training delivery, and supportive culturally and linguistically tailored activities

Minimum Qualifications:
A high school diploma and five years of appropriate experience required.
Valid Driver's License required.
Preferred Qualifications/Skills:
Bi-lingual.
Bachelor's/master's degree from an accredited institution in an appropriate field of study.
Salary:
$20.00 - $25.00 per hour.
