    Part-Time, Monday - Friday

     

    Pay Rate: $45.00/hour

     

    Expected Start Date: Early-Mid July

     

    JOB SUMMARY: The speech language pathologist will develop and implement treatment plans, evaluate and treat residents. The speech language pathologist will provide home recommendations to residents for discharge, as needed.

     

    POSITION REPORTS TO:Director of Rehabilitation

    ESSENTIAL JOB FUNCTIONS

    + Evaluate, diagnose and treat language, voice, speech and swallow and cognitive disorders of the residents.

    + Utilize professional judgment, the admission notes of the attending physician and/or the physician or nursing request when treating the resident.

    + Establish appropriate short and long-term goals for the resident.

    + Document specific treatment plan, integrate treatment with needed disciplines and provide treatment as indicated.

    + Prepare and maintain appropriate resident treatment schedules.

    + Document initial goals, progress notes, treatment results and any recommendations.

    + Modify patient care goals and terminate treatment, as goals are achieved or unobtainable.

    + Meet customer service standards in a friendly, helpful, courteous manner, with resident satisfaction, respect and dignity as priorities.

    + Respect residents’ rights to privacy, confidentiality and opportunity to choose in matters that affect them.

    + Participate in or support ongoing quality improvement programs.

    + Conduct in-service training for professional and non-professional staff, residents and families as appropriate.

    + Participate actively in the quality improvement procedures of the department.

    + Participate in committee and administrative meetings, as requested.

    + Comply with facility in-service requirements.

    Requirements

    Maintain confidentiality of all information related to the organization, residents, participants, family and staff, that may be encountered, either formally or informally, during the normal course of business. This includes medical and treatment records, financial and human resources information.

     

    QUALIFICATIONS:Professional expression and execution of duties relative to assignments.

     

    Good oral and written communication skills. Ability to work effectively with other health team members. Ability to interact courteously and tactfully with staff, residents, family members, visitors, vendors and the general public.

     

    EDUCATION:Licensed and currently registered as a speech language pathologist by the State of New York.

     

    EXPERIENCE:2 yearsLong-term care experience preferred.

    Performing duties of this job requires occasional sitting, walking and standing. Must be able to occasionally lift loads of 30 pounds without assistance and the ability to sit, talk, and hear is required. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, color vision, and the ability to adjust focus.
