Colorado Veterans Community Living Center (VCLC) at Fitzsimons is a beautiful 180-bed, skilled nursing facility serving Colorado’s veterans. At Fitzsimons, we provide long-term care and short-term rehab and have a secured memory care neighborhood. Description of Job
The work unit exists to provide quality clinical and distributive pharmacy services for residents receiving medications from the Colorado Veterans Community Living Center Pharmacy. Performs comprehensive assessments and analysis of drug therapy in collaboration with resident care team members. Accountable for ongoing evaluation and documentation of all components of pharmaceutical care delivered to residents. Train and mentor the activities of the pharmacy I positions, oversee pharmacy technician positions, coordinate workflow, and checking all medications prior to being dispensed. The pharmacist will be in charge and the main point of communication regarding cycle fill for various facilities.
This position performs day-to-day professional pharmaceutical operations of the department, which includes drug acquisition, distribution, and monitoring, identifying, resolving, and preventing drug-related problems that may affect optimal resident care outcomes, and to research and communicate with our medical and nursing staff, as well as all ancillary services.
Additionally, as a Pharmacist II you can expect to:
+ Direct procurement, storage, distribution, control, and evaluation of drugs
+ Formulates specifications for the quality and sources of all drugs procured by the pharmacy
+ Dispenses and compounds prescribed medications, drugs, and other pharmaceuticals/supplies for resident care, according to professional standards and State and Federal legal requirements
+ Compounds medications using standard formulas and processes, such as weighing, measuring, and mixing ingredients, making and checking IV admixtures, blister packs, and restocking emergency kits
+ Reviews prescriptions issued by physicians or other authorized personnel to assure accuracy, and determine formulas and ingredients needed
+ Assay medications to determine identity, purity, and strength.
+ Oversees the manufacture of bulk compounded preparations if their procurements from commercial sources are not possible
+ Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs
+ Inspects stock supplies of drugs on all services.
+ Maintains approved stock of antidotes and other emergency drugs. Communicates with clinical staff regarding medications and changes
+ Oversees, trains, orientates, and leads/directs pharmacy technician staff in filling, labeling, and dispensing of all drug orders
+ Directs medication cart/room/room/refrigerator audits
+ Enters data such as resident name, prescribed medication, and cost to maintain pharmacy files, insurance system, and inventory
+ Reviews resident charts, making recommendations, dosing, and addressing drug interactions
+ Answers questions and provides information on drug interaction, side effects, dosage, lab evaluation, and storage of pharmaceuticals
+ Run reports and share them with associated facilities to notify them of what medications are being cycle filled
+ Recognize and send which prescriptions require refill requests and are being followed up on
+ Perform drug utilization review to determine accuracy
+ Maintains updated skills, knowledge, abilities, and competencies necessary through continuing education, clinical supervision, literature review, and participating in discipline and team development
+ Other duties as assigned
Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
Minimum Qualifications:
Education/Licensure/Certification/Experience:
+ Current, valid licensure as a Pharmacist by the Colorado State Board of Pharmacy
AND
+ Two (2) years of experience in an occupational field related to the work assigned to the position
Preferred Qualifications:
+ Advanced Excel skills, including complex formulas, pivot tables, and data visualization
+ Detail-oriented with strong organizational skills
+ Knowledge of basic pharmacy practices and procedures
+ Retail Pharmacy or Long Term Care Pharmacy experience preferred
CONDITIONS OF EMPLOYMENT:
+ CDHS employees (all Direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adult Mental Health Institutes, Regional Centers, Veterans Community Living Centers)
+ Current/Valid and unrestricted US Driver’s License
+ May have to transport medications to other VCLC facilities in state vehicles
+ Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
