Washington County Public Health is seeking a talented and motivated public health professional to join our team as a Community Health Specialist/Senior Community Health Specialist. The Community Health Specialist works to develop, promote, and evaluate community-based initiatives aimed at health promotion, the prevention of disease and injury, and community engagement. This position will primarily work with partners in the schools settings on topics related to nutrition, physical activity, commercial tobacco prevention, and mental well-being. This position will also focus on substance use prevention efforts.
POSITION SPECIFIC JOB DUTIES
+ Provide project management, technical assistance, and resources for health promotion initiatives in the schools setting for students and staff in the areas of nutrition, physical activity, commercial tobacco prevention, and mental well-being
+ Support population-level prevention efforts to reduce substance use among key populations (e.g. youth, pregnant and breastfeeding individuals, and communities experiencing greater health disparities)
+ Using data and community engagement efforts, develop and implement policy, systems, and environmental changes to promote community health and reduce existing health disparities
+ Coordinate partnerships with schools, healthcare systems, community organizations, and individuals with lived experience to advance culturally responsive, evidence-based prevention efforts
+ Conduct and support assessment efforts including data review, conducting focus groups, and identifying top health priorities with a health equity lens
+ Work alongside community partners to develop and implement projects and action plans to address community health priorities
+ Manage partner projects and grants, including communicating with partners, identifying goals and outcomes, supporting implementation, evaluating projects, and tracking budgets
+ Monitor and evaluate activities related to program area
+ Participate in and/or facilitate coalitions of community agencies, organizations, groups, and individuals to understand and promote prevention efforts
+ Present and share data and other key program information to partner organizations and stakeholders
+ Create and maintain partnerships with businesses, schools, healthcare providers, law enforcement, community agencies, and community members to develop and support programs
+ Apply a consistent health equity lens to all prevention activities, ensuring strategies are inclusive, culturally responsive, and accessible to engaged communities
+ Participate and represent the department and/or the county on various working groups as assigned
DESIRABLE EXPERIENCE :
+ Knowledge of public health systems including policy, systems, and environmental change strategies to promote health and reduce chronic diseases
+ Knowledge of the spectrum of prevention and health promotion practices
+ Knowledge of and experience with project management and group facilitation
+ Experience and deep understanding of cultural competency, diversity, inclusion, and health equity in order to reduce health disparities

HOURS / DAYS: Position is based at the Washington County Government Center in Stillwater, with Telework options available. Work hours are primarily Monday through Friday between the hours of 8:00 am – 4:30 pm. Evening and weekend hours may be required. Work location and hours are subject to change based on business need.

JOB SUMMARY
The Community Health Specialist works to develop, promote and evaluate community based initiatives aimed at health education, promotion and maintenance, the prevention of disease and injury and community development.
EMPLOYMENT CRITERIA
MINIMUM QUALIFICATIONS: The job requires a Bachelor's Degree in health education, nursing, nutrition or directly related field, or equivalent.
PREFERRED QUALIFICATIONS: A Master of Public Health degree and Certification as a Health Education Specialist is desirable.

DUTIES & RESPONSIBILITIES
Manage community based health promotion initiatives:
• Provide leadership to community based organizations to plan, implement and evaluate programs or promotions;
• Provide development of short and long-term goals and action plans to address community health priorities;
• Advocate for public, organizational and community policies (local, regional and state) that create supportive and healthy environments;
• Interpret federal, state and local policies, regulations and rules for staff and community members;
• Recruit, train and coordinate community members to help implement local community health initiatives;
• Organize education, audiovisual, and publicity materials;
• Compile and prepare statistics and narratives or annual reports, Community Health Plans, grants and reports, program evaluations and other documents;
• Monitor health promotion initiatives for legal compliance, policies ordinances, procedures, reporting and budget requirements; and
• Make budget, staffing and service level recommendations to meet program goals.
Plan, implement and coordinate community health assessments:
• Collect and analyze behavioral, environmental and epidemiological data to evaluate community health issues;
• Prepare and present health data and statistical summaries for public forums, agencies, health organizations, schools and community groups;
• Create partnerships with businesses, schools, healthcare providers, law enforcement, community agencies and community members to develop and support programs; and
• Provide consultation, facilitation and education to individuals and groups for specific health priorities such as nutrition consultations, alcohol compliance, behavioral health, and chronic disease.
Collaborate with government agencies, health care providers, community organizations, schools, businesses, and the community.
Perform other duties as assigned.
WORK ENVIRONMENT
Work performed is semi-sedentary in nature, and typically performed within the Government Center, a satellite office or other location. Lifting requirement of up to 20 pounds on an occasional basis. Equipment used may include, but is not limited to, computer, calculator, telephone, camera, and copy and fax machine.
Work interruptions are frequent. There is some pressure associated with dealing handling controversial issues or group of individuals with differing opinions, project or activity deadlines and details.
Must have means of reliable transportation for business purposes; individuals operating a motor vehicle for county business must possess a valid driver's license.
