    The DEMP Program Manager is responsible for leading the quality improvement, coordination, and clinical innovation of people with diabetes across the care continuum. The individual is responsible for managing the processes, protocols, and policies around diabetes care. This individual supervises all staff and is responsible for managing the program’s data, and any program outcome deliverables associated with quality, safety, access, patient experience, operational, and financial measures. Serves as a subject matter expert, facilitates interprofessional collaboration, and promotes program objectives while ensuring compliance with appropriate regulatory agencies.

    + Identifies potential and actual risks and appropriately escalates as needed to mitigate or resolve.

    + Lead and manage a systemic, collaborative, and coordinated approach to quality measurement and improvement initiatives.

    + Utilizes Lean methods and a variety of Quality Assurance and Performance Improvement (QAPI) tools.

    + Leads and actively participates in QAPI projects.

    + Collaborates with senior leaders to identify and develop strategic goals and plans.

    + Manages regulatory site visit preparation materials and documents as applicable.

    + Provides stroke education opportunities for clinical staff.

    + In addition to the above job responsibilities, other duties may be assigned.

    MINIMUM REQUIREMENTS

    **Education:** Bachelor of Science in Nursing from accredited nursing program. Master’s degree in related field within 4 years.

    **Experience:** Three (3) years progressive experience and relevant clinical nursing experience required.

    Specialty Board Certification strongly preferred

    **Licensure:** Must be a registered nurse and currently licensed to practice in the Commonwealth of Virginia.

    PHYSICAL DEMANDS

    This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.

     

    Position Compensation Range: $104,062.40 - $138,819.20 Annual
