Program Description:
A ´Health Home´ is not a physical place; it is a group of health care and service providers working together to make sure children get the care and services they need to stay healthy. Once they are enrolled in a Health Home, they will have a care manager that works with them to develop a care plan. A care plan maps out the services they need, to put them on the road to better health. Some of the services may include:
+ Connecting to health care providers,
+ Connecting to mental health and substance abuse providers,
+ Connecting to needed medications,
+ Help with housing,
+ Social services (such as food, benefits, and transportation) or,
+ Other community programs that can support and assist you.
Principal Objective/Role Overview:
The Administrative Assistant works to primarily support the program’s administrative requirements. This role is pivotal to the program; this staff member will efficiently attend to a range of responsibilities, such as creating segments in MAPP, assisting supervisors with billing concerns in both MAPP and EHR, conducting surveys with enrolled families, assisting Care Managers with DocuSign and other tasks.
Job Responsibilities:
+ Prepare HH consents for enrollment and once signed consents are reviewed to ensure consents are signed correctly. Upload consents to FCM.
+ Support Intake Care Manager with outreach (i.e. obtaining documents, checking Medicaid numbers, keeping consents updated in shared drive and in Foothold (EHR))
+ Complete all enrollments and disenrollments in MAPP/Foothold and keep Health Home tracker updated.
+ Send disenrollment letters to all members via email/mail once disenrolled.
+ Update MAPP/Foothold segments and UAS profiles when a member has a CIN change. Communicate with Foothold to create special MAPP segments for children over 21.
+ Verify that all Medicaid CINS are active and alert team if they are not
+ Troubleshoot Medicaid and CANS errors.
+ Send monthly CANS reports to analyst team.
+ Send weekly CANS reports that were finalized and in queue to care managers and supervisors to ensure the correct lead HH and assessment type were chosen to prevent billing issues.
+ Conduct 10 surveys per month.
+ Use EPIC/Netsmart/PSYCKES to locate and upload clinical documentation annually for enrolled members to satisfy eligibility requirements.
+ Assist Care Managers with obtaining signatures from caregivers for plans of care, enrollment/disenrollment consents and other documentation.
+ Alert families of upcoming Medicaid recertification
Qualifications
Position Qualifications:
+ Minimum of Associates Degree/Bachelor’s Degree required upon application
+ Minimum of 2 years’ experience in office management & client relations.
+ Detailed-oriented.
+ Excellent communication skills.
+ Excellent computer skills
+ Bilingual Spanish Speaking preferred.
Salary & Compensation:
+ Base Salary or Hourly Rate: $25.15/Hour ($45,771.92/year)
+ FLSA Status: Non-Exempt
