**Schedule:** Full Time

**Shift** : Monday-Friday 8am-5 pm

**Location:** Bartow, Floyd, Gordon counties

The role you'll contribute:

Executes physician's orders and notifies physician of necessary updates or changes in a home health patient's condition. Assesses, plans, implements, and evaluates the individualized occupational therapy care provided to a patient. Responsible for the coordination of care of all assigned patients. Deliver high quality and professional occupational therapy care. Continued employment with the agency is contingent upon your ability to perform job duties as outlined in this job description.

+ Instructs patients and families in the total occupational therapy program, including self-care and creative activities aimed at improving independence, physical, and mental functioning.

+ Establishes household management routines to support patient independence.

+ Participates in the team process through cooperative efforts with team members in developing the patient care plan and attending team conferences.

+ Other duties as assigned.

+ Supervises and plans home health aide activities in the absence of other healthcare professionals.

+ Demonstrates effective communication skills with all team members, patients, and families.

The expertise and experience you'll need to succeed:

Minimum qualifications:

+ 1 year work experience

+ Occupational Therapist (OT)

+ Active American Heart Association BLS

+ Valid in state Drivers License with current car insurance

Preferred qualifications:

+ Master's degree

+ 1 year of clinical experience as an Occupational Therapist in Home Health
