    Dementia Program - APP Opportunity

    Position Details

    + Part-time Licensed APP to support the development and growth of our Dementia Program

    + Outpatient-based care, with no call responsibilities

    + Travel to patient homes within the Flathead Valley, as needed

    + Some telehealth services may be incorporated

    + Collaborative team environment—working closely with a Nurse Navigator and Medical Assistant

    Preferred Qualifications

    + 1–2 years APP experience preferred

    + Experience with geriatric or memory care populations strongly preferred
