The Family Partner works as a member of the treatment team to support youth and family by assessing their needs, linking them to network and community resources, assisting with developing and monitoring the implementation of treatment and aftercare plans, and addressing the barriers which may be interfering with member’s desired outcomes for a community involved quality life. Populations served by The Hope Center for Youth and Family Crisis may include a variety of specialty health care groups that require long term including but not limited to people living with mental health and substance use conditions, intellectual and/or developmental disabilities, and survivors of traumatic brain injuries.
QUALIFICATIONS:
Experience in navigating any of the child and family-serving systems and teaching family members who are involved with the child and family serving systems.
High school diploma or GED and a minimum of one year of experience working with children/adolescents/transition age youth and families.
Must have lived experience as a primary caregiver for a child who has/had mental health or substance abuse challenges and identifies as such.
National Certification in Family Peer Support or is actively working on completing certification and is on track to complete Family Peer Support certification within one year of hire date. Review link http://www.ffcmh.org/certification for complete expectation.
Must attend 12 hours of Person Centered Thinking training prior to or within 90 days of hire.
The candidate must pass the criminal record history clearance and a child abuse clearance as well as possess a valid driver's license.
LSCI, SCM, Ukeru training as appropriate for position.
JOB DUTIES:
Build partnerships with public and private mental health agencies, child serving agencies, DSS, DJJ and community partners to value and invest in family involvement activities in an effort to sustain family involvement through the establishment of natural supports.
May be assigned to work as a responder for Mobile Outreach Response Stabilization and Evaluation team lead, providing crisis services in Wake and surrounding counties.
Provide outreach to family members of children who have received services with wellness calls within 72 hrs of discharge from care.
Work in partnership with the center team to implement the day–to–day activities of the grant and participate in all administrative team meetings and discussions.
All other duties as assigned by management.
