(We have multiple Community Habilitation Worker positions available acrossvarious locations in Brooklyn,each with a different schedule. Availability includes weekdays, evenings, and weekends. Let us know your preferred location and schedule when applying!

Pay Rate:$19.00/hr.

Hours:3-20 per week

Community Habilitation Worker

Join our team and make a difference! As aCommunity Habilitation Worker, you’ll empower individuals with developmental disabilities by promoting independence, personal choice, and community integration.

Key Responsibilities

+ Provide person-centered support in homes and the community.

+ Assist individuals in building life skills, relationships, and self-advocacy.

+ Ensure safety, well-being, and adherence to individual care plans.

+ Encourage community involvement and inclusion.

+ Maintain accurate documentation and uphold compliance standards.

+ Communicate effectively with individuals, families, and team members.

+ Participate in training and professional development.

Qualifications

+ High School Diploma or GED.

+ Strong communication and interpersonal skills.

+ Ability to work independently and as part of a team.

+ Basic computer skills for documentation and communication.

+ Valid driver’s license preferred.

+ Bilingual in English & Bengali is a plus!

Work Environment & Physical Demands

+ Community-based and home settings.

+ Ability to perform physical tasks such as standing, lifting, and walking.
