• About the RoleWe’re seeking compassionate and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide personalized care in a home setting. If you’re passionate about making a direct impact on patients’ lives and thrive in a collaborative environment, this is your chance to join a dedicated team of healthcare professionals.🧠 Responsibilities

+ Deliver patient care and treatments in alignment with individualized care plans

+ Promote health through education, counseling, and preventive care

+ Administer medications and tailored interventions

+ Collaborate with multidisciplinary teams to coordinate care

+ Maintain accurate daily nursing documentation

+ Assist with mobility and physical support (must be able to lift up to 50 lbs)

+ Remain active and engaged during extended periods of standing or movement

+ Take on additional duties as needed to support patient well-being

• Qualifications

+ Minimum 1 year of experience as an LPN or RN in a home care setting

+ Active LPN or RN license issued by New York State

+ Strong communication and organizational skills

+ Empathy, reliability, and a commitment to high-quality care
