We’re hiring Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide personalized, 1:1 care across Suffolk County.What You’ll Do:

+ Deliver direct patient care and treatments based on individualized care plans

+ Administer medications and tailored interventions

+ Promote wellness through education, counseling, and preventive care

+ Collaborate with multidisciplinary teams to coordinate care

+ Maintain accurate and timely nursing documentation

+ Assist with mobility and physical support (must be able to lift up to 50 lbs)

+ Stay active and engaged during extended periods of standing or movement

+ Take on additional duties as needed to support patient well-being

What You’ll Need:

+ Active LPN or RN license issued by New York State

+ Minimum 1 year of experience in a home care setting

+ Current CPR/BLS certification

+ Strong communication and organizational skills

+ Empathy, reliability, and a commitment to high-quality care
