    Amergis (Crestwood, IL)


    Salary: $2437 / Week

    DLP Program:

    + Located in Midlothian and Crestwood, IL, EC-8th Grade

    + Ave caseload 20-25 students, Ave weekly minutes is 1100-1200

    + Students have severe to profound autism, and intellectual, physical, multiple disabilities

    + Hours 8:10-2:40 M-F, Paid for 6 hours

    DHH Program:

    + Located in Crestwood, IL, EC-HS

    + Ave caseload 20-25 students (will be adjusted for part time), Ave weekly minutes 1100-1200 (will be adjusted for part time)

    + Students are deaf and hard of hearing

    + Hours 8-3, Paid for 6.5 hours

    + Sign language preferred but not required

    *THIS IS ONE POSITION, TWO PROGRAMS COMBINED FOR TOTAL OF 32.5 HOURS*

     

    The Speech-Language Pathologist in the school setting isresponsible for performing student evaluations as well as providing anddocumenting therapy services in accordance with the plan of care developed foreach individual student and the physician's orders.

    Minimum Requirements:

    + Active SpeechLanguage Pathologist (SLP) Licensure in the state of assignment required

    + Certificate of Clinical Competence in Speech-LanguagePathology (CCC-SLP) preferred and may be required by state/contract.

    + Complies with all relevant professional standards ofpractice

    + Current CPR if applicable

    + TB questionnaire, PPD or chest x-ray if applicable

    + Current Health certificate (per contract or stateregulation)

    + Must meet all federal, state and local requirements

    + Must be at least 18 years of age
