Position Summary
The Psychosocial Rehabilitation (PSR) Specialist provides individualized, hands-on support to help children and youth develop age-appropriate skills for daily living, social interaction, and community integration. Services are goal-driven and rooted in each youth’s treatment plan, with the aim of reducing functional deficits and promoting long-term independence with minimal professional intervention. All services are delivered in a trauma-informed, culturally and linguistically responsive manner and are recommended as part of an individualized treatment plan by a licensed practitioner. This is a community-based position.
Key Responsibilities
Skill Development and Support
+ Coach youth in social and interpersonal skills to strengthen relationships, build community awareness, develop support networks, and increase resilience in home, school, and work environments.
+ Teach coping strategies for stress, unexpected events, and the management of behavioral and physical health symptoms.
+ Support youth in creating and maintaining friendships, enhancing social etiquette, and practicing anger management.
Daily Living and Autonomy
+ Help youth develop routines that promote independence, including self-care, medication management, and constructive engagement with healthcare providers.
+ Encourage healthy habits and relapse-prevention strategies while connecting youth to community resources.
Community Integration
+ Guide youth in applying learned skills to real-world settings, maintaining developmentally appropriate functioning at home, school, and in the community.
+ Assist with identifying triggers and practicing strategies to manage or avoid functional impairments.
Program Participation and Documentation
+ Collaborate closely with CPST, OLP and other providers to ensure coordinated care..
+ Complete accurate progress notes, treatment planning and all applicable documentation.
+ Track and submit all required paperwork, service units, mileage, and schedules within established deadlines.
General Responsibilities
+ Maintain confidentiality and adhere to mandated reporting requirements.
+ Participate in supervision, trainings, workshops, and division-wide meetings.
+ Safely transport youth using agency or personal vehicles as needed, following all agency safety policies.
+ Perform other related duties as assigned.
Qualifications
+ Minimum 18 years of age.
+ High school diploma or equivalent required; State Education Commencement Credential preferred.
+ At least two years of experience in children’s mental health, addiction services, foster care, or a related human services field.
+ Demonstrated understanding of childhood psychiatric disorders and commitment to family-driven, youth-guided, strength-based care.
