Position Summary:

The primary responsibility for this position will be interpreting Behavioral Health visits.

The secondary role is as a Behavioral Health Care Manager, working closely with patients' primary care providers in clinic and/or school-based settings. This role involves coordinating referrals, supporting behavioral health services, and ensuring adherence to state and federal confidentiality requirements. You will work under the direction of licensed behavioral health and medical providers to help patients access the care they need.

Employment Type: Full-Time (40 Hours per week)

Salary (Wage): $28.00 - $30.00/hour

Key Responsibilities:

+ Interpret: Provides interpretation for patient and provider team.

+ Coordinate: Support and coordinate behavioral health services with the patient’s integrated care team.

+ Engage : Connect with patients during warm hand-offs to offer support, education, and resources.

+ Facilitate : Assist patients in accessing services using approved multi-media platforms and ensure follow-up care.

+ Track: Monitor patient follow-up and clinical outcomes, documenting encounters in the EHR.

+ Refer: Facilitate referrals for clinically indicated services specified on the patient’s treatment plan.

+ Support: Assist with behavioral health screenings and intake processes.

Qualifications:

Education

Required:

+ Associates degree or in lieu of degree Four (4) years' experience in community health setting

+ Valid driver’s license

+ BLS Certification upon hire

+ Bilingual in English/Spanish per One Health's Internal testing process

Preferred:

+ If English as second language, TOEFL test score between 50-100 in the last 10 years

Experience

Required:

+ One year using English/Spanish in a healthcare setting or one year of case management experience.

+ Proficient computer skills to include internet-based applications

+ Must be a member of the community served and/or demonstrate involvement in community served

Preferred:

+ Three (3) years of case management experience in health care setting and/or mental health setting.

+ Interpretation experience in a medical or legal setting
