    The RN Care Delivery Manager is responsible for supporting nursing standards of excellence through advanced professional education to deliver high-quality care. This role serves as a member of the management team, working collaboratively with all departments and employees to oversee day-to-day operations. The position provides guidance and implements best practices to achieve the organization's objectives, goals, and mission. The role fosters an environment dedicated to improving experience, enhancing satisfaction, and retention.     Responsibilities

    + Review and ensure effective development and execution of personalized care plans that result in improved satisfaction and outcomes for all participants.

    + Facilitate and complete the process of Interdisciplinary Team (IDT) meetings, Service Delivery Requests (SDR) and other meetings, including completing attestations as needed.

    + Serve as a manager on call during non-operational hours of physical sites and handle follow-ups including documentation.

    + Balance and allocate staffing and resources throughout the site to ensure adequate team assignments and seamless operation.

    + Ensure documentation for participants is complete and accurate, including enrollment, disenrollment, and clinical notes.

    + Collaborate with the Transitions in Care Team to provide care management services across sites and communicate with team members, facilities, and coordinators during care transitions.

    + Ensure documentation complies with CLHC standards and federal/state regulations.

    + Collaborate with Human Resources for interviews, coaching, and disciplinary actions. Assist in onboarding and professional development training of staff.

    + Evaluate the work performance and competency of direct reports, providing feedback and completing annual performance reviews.

    + Act within the scope of the individual's authority to practice.

    Essential Skills

    + Current active and unrestricted RN license and registration in New York State.

    + Minimum of two years of administrative experience in a management capacity in healthcare settings.

    + Supervisory skills and management skills.

    + Be legally authorized to practice in the state where healthcare services are performed.

    + Medically cleared for communicable diseases and up-to-date immunizations.

    Additional Skills & Qualifications

    + Bilingual language skills are preferred.

    + Care management and case management experience.

    + Customer service experience is required.

    + Experience with managed long-term care insurance is beneficial.

    + Experience working with frail or elderly populations, or appropriate training from the PACE organization.

    Work Environment

    The position is based in an onsite medical clinic within a community healthcare setting. The work hours are from 8:30 AM to 5:00 PM, totaling 37.5 hours per week.     Pay and Benefits

    The pay range for this position is $120000.00 - $125000.00/yr.
