Job Title: PROS Clinician, Licensed Practitioner of the Healing ArtsDepartment: Personalized Recovery Oriented Services Location: 224 Alexander St, Rochester, NY 14607 Hours Per Week: 40Schedule: 8:00AM - 4:30PM Sign-On Bonus: $10,000 SUMMARY:The PROS Clinician, LPHA is a member of the PROS (Personalized Recovery Oriented Services) and provides diagnostic assessment, individual recovery planning, medication monitoring, individual and group therapy for PROS participants who are enrolled in the PROS program. Additionally, the PROS Clinician, LPHA (Licensed Practitioner of the Healing Arts) is authorized by clinical scope of practice and licensure to approve PROS admissions and recovery plans for participants assigned to clinicians who do not have a designation as LPHA.
RESPONSIBILITIES:
+ Provides clinical counseling and psychotherapy to designated caseload. Provides PROS participants support for the achievement of recovery outcomes (i.e., employment). Provides individual, group, and family psychotherapy, and monitors participants’ response to psychotropic medications. Provides Outreach and Engagement when necessary.
+ Develop/facilitate class curriculum for skill building classes.
+ Participates as a member of the interdisciplinary PROS team in coordination and delivery of care. Communicates with relevant internal and external care providers to complete ongoing assessment and to coordinate care. Demonstrates knowledge of and linkage with community and natural supports.
+ Participates in Quality Improvement activities within the PROS program and department.
+ Participates in and incorporates professional supervision and ongoing education toward quality care, especially Evidence Based Practices (COD, Family Psychoeducation, etc.).
+ Completes all required documentation in a timely and comprehensive manner.
+ Coordinates care with primary medical providers and others in the community.
+ Maintains a full caseload, meeting productivity requirements and seeks to keep the incidence of failed appointments to a minimum.
+ Individual Recovery Plans – Incorporate individual strengths, aspirations, barriers from assessments and reflect participants’’ full input. Participants on assigned caseload have clearly defined recovery goals which are stated in their own words.
+ Uses recovery and person centered approaches, including person first language when working with consumers and families. Provides ongoing support for participants for goal acquisition (i.e., employment).
+ Provides clinical consultation/supervision to the clinical team as delegated. Completes all co-signature sign-off (LPHA) as required/requested and within designated timeframes (According to OMH Part 512).
+ Delivers PROS clinical counseling, psychotherapy and symptom monitoring and documents in accordance with BHN policy.
+ Demonstrates ability to maintain a dynamic caseload.
+ Demonstrates ability to determine medical necessity of care for patient.
+ Demonstrates safe driving practices in accordance with Rochester Regional Health driver policy.
REQUIRED QUALIFICATIONS:
+ LMSW, LCSW, or LMHC required (Active Registration NYS), Licensed Practitioner of Healing Arts required.
+ Valid New York State driver’s license preferred.
+ Approved driving record – No Major or Capital violations within the last 5 years.
+ Completion of Mandatory Driver Education (upon employment).
+ Participation in NYS Department of Motor Vehicles License Event Notification Service (LENS)(upon employment)
PREFERRED QUALIFICATIONS:
+ Master’s Degree in Social Work, Mental Health Counseling, or a degree that would be considered by the office of professions for one of the required licenses below.
+ One year of experience preferred.

$66,000.00 - $86,000.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
