    Description

    Position Summary:

    Day 1 Families Fund Case Manager will be responsible for assisting families in securing and maintaining stable housing while also providing life skills education to support long-term housing retention and family well-being. This position combines case management, housing navigation, and education functions. The Case Manager & Educator will meet with clients to assess housing needs and barriers, develop housing stability plans, and connect families to resources. In addition, this position will conduct workshops and one-on-one sessions in areas such as budgeting, employment readiness, computer skills, and parenting.

    Essential Job Functions:

    + Housing Navigation & Case Management

    + Manage a caseload of up to 30 participants.

    + Assess client housing needs, barriers, and eligibility for housing programs.

    + Assist with securing necessary documentation, completing applications, and monitoring housing progress.

    + Provide warm handoffs to supportive services and community resources.

    + Ensure smooth transition and retention in permanent housing.

    + Participate in Fresno Madera Continuum of Care (FMCoC) case conferencing, trainings, and CES updates.

    + Provide weekly updates to the Master-By-Name List.

    + Education & Life Skills Development

    + Facilitate workshops for families in life skills, personal budgeting, job readiness, parenting, and other relevant areas.

    + Provide one-on-one coaching and support to promote family self-sufficiency.

    + Create a safe, engaging, and supportive learning environment.

    + Encourage active participation from parents and children.

    + Conduct outreach in the community to connect individuals with resources and offer classes.

    + Documentation & Compliance

    + Maintain accurate case notes, reports, and documentation in compliance with program, state, and federal guidelines.

    + Collect and report data for program evaluation and outcome tracking.

    + Ensure services are delivered in a culturally and linguistically appropriate manner.

    + Other Duties

    + Represent the agency in community meetings as requested.

    + Support research and program evaluation efforts.

    + Embrace and embody the mission, vision, and values of WestCare Foundation.

    + Perform any other duties as assigned.

    Qualifications

    Essential Qualifications:

    + Minimum two (2) years’ experience in social services, case management, or related human services field.

    + Strong knowledge of housing programs, supportive services, and community resources.

    + Ability to design and implement life skills curricula.

    + Excellent communication, public speaking, and interpersonal skills.

    + Strong computer skills, including Microsoft Office (Word, Excel, PowerPoint).

    + Good documentation and organizational skills.

    + Ability to work independently, manage multiple priorities, and remain calm in challenging situations.

    + Demonstrated ability to work effectively with diverse and underserved populations.

    Certifications/Licenses:

    • Valid driver’s license and ability to transport clients.

    • CPR Certification, First Aid Certification, and annual Tuberculosis test.

    Education:

    + Graduation with a two-year degree in Psychology, Social Work, Community and Human Services, Rehabilitation Counseling or a related field is required; and

    + High School diploma or equivalent is required.

    Experience and Competencies:

    Working Conditions:

    + Work is primarily performed in an office setting;

    + Some travel is required; and

    + Some outdoor activities may be required

    The employee must be able to perform the following essential duties and activities with or

    Physical Demands:

    + Occasional lifting of more than 10 pounds;

    + Sitting, standing, walking, reaching are performed in the normal course of the position; and

    + The normal work routine involves no exposure to blood, body fluids, or tissues, BUT exposure or potential exposure may be required as a condition of employment. Appropriate personal protective equipment will be readily available to every employee engaged in Category II tasks;

    M ental Demands:

    + Requires the ability to collect and analyze complex numerical and written data and verbal information to reach logical conclusions.

    + Requires the ability to work and cooperate with clients, co-workers, managers, the public and employees at all levels in order to exchange ideas, information, instructions and opinions.

    + Requires the ability to work under stress and in emotionally charged settings.

    + The ability to defend oneself and clients in mentally/verbally abusive situations through the use of approved mental/verbal de-escalation techniques.
