    Work Schedule: 8 hours variable

    Summary:

    The clinical gastroenterology pharmacist will primarily practice in the ambulatory setting and work with other members of the gastroenterology team, in collaboration with Northern Light’s specialty pharmacy and infusion centers. This candidate will work to deliver comprehensive care for patients on oral and injectable medications, providing a wide variety of services to patients, including education, adverse event management, evaluating drug—drug and drug–disease interactions, and aiding in obtaining financial coverage of these medications. This position will focus on patient management, medication adherence, and collaboration with other members of the healthcare team to ensure optimal patient outcomes. This position will provide patient counseling for patients newly started on gastroenterology-related medications. They will create and maintain evidence based collaborative practice agreements for the management of these patients, as applicable. They will work on an interdisciplinary medical team to ensure optimal medication management for all gastroenterology patients. Tasks will include providing in-office and phone follow-up care to gastroenterology patients, completing documentation and billing processes for pharmacist activities, maintaining up to date clinical knowledge, ensuring practice and department follow accreditation guidance for specialty pharmacies, and building and maintaining relationships to ensure access to specialty medications, as well as other duties as assigned.

    Responsibilities:

    + Monitor patients for adherence and toxicity management and navigate patients through their treatment.

    + Serve as a drug information resource for providers, nurses, and patients.

    + Provide education to providers, nurses, patients, and students.

    + Ensure compliance with specialty pharmacy accreditation and develop/maintain collaborative practice agreements/protocols.

    + Support pharmacy liaison as applicable.

    + Other Duties as assigned.

    Other Information:

    + Board Certification in Ambulatory Care Pharmacy or Pharmacotherapy, alternatively three years in a similar practice setting.
