LPN | Licensed Practical Nurse – Sacred Heart Home Assisted LivingExperienced nurses can earn up to $27.25/hr

What You’ll Do:
+ Provide and evaluate direct nursing care for residents
+ Serve as Charge Nurse, supervising PCAs
+ Support personal hygiene, nutrition, mobility, and restorative care needs
+ Transcribe physician’s orders and assist with medical rounds
+ Observe and report resident condition changes to RN Supervisor or physician
+ Assist with care plans and coordinate multidisciplinary care
+ Administer medications safely, including controlled substances
+ Order, document, and manage medications and lab tests as needed
+ Arrange transportation for resident appointments
+ Ensure comfort and safety measures are applied consistently
+ Accurately document and communicate resident care activities

Qualifications
+ CurrentNew York State LPN licenserequired
+ Previous nursing experience in assisted living, long-term care, or related setting preferred
+ Strong clinical assessment and documentation skills
+ Leadership ability to supervise and support PCAs
+ Compassionate, dependable, and dedicated to quality resident care
