    ### Job Duties

    This position will be responsible for planning intensive care coordination for people wanting to remain in their home or relocate to the community after a stay in a care facility. This position is work from home, preferably in Ottertail, Wilkin, Traverse, Grant, Stevens, or Pope counties. Regular travel required, including throughout the 21-county service area.

     

    ### Minimum Education Required

    Minimum of a bachelor’s degree from an accredited college or university in Nursing, Social work, Gerontology, or a related human services field.

    ### Minimum Experience Required

    Experience working in person-centered discharge planning, charting, care coordination, or related tasks. Proficient in problem-solving skills, adept at phone etiquette, and strong in computer skills.
