    Licensed Practical Nurse (LPN)

    Responsibilities:

    + Provide hands-on nursing care and support for patients with chronic conditions like diabetes, HIV, hypertension, and obesity.

    + Coordinate care for elderly and disabled individuals, ensuring they receive the attention and resources they need.

    + Travel between multiple care sites within Fairfield County (no overnight travel required).

    + Work closely with a multidisciplinary team to deliver comprehensive, patient-centered care.

    + Manage referrals and assist patients in navigating their healthcare journey.

    Must Haves:

    + Active LPN license in Connecticut.

    + Minimum 2 years of experience in a primary care or outpatient setting.

    + Experience working with high-risk or chronically ill populations.

    + Proficiency with Electronic Health Records (EHR).

    + Strong communication skills and a collaborative mindset.

    + Bilingual in English and Spanish

    Skills to Have:

    + Familiarity with population health and referral coordination.

    + Excellent time management and organizational skills.

    + A compassionate, detail-oriented approach to care.

    Work Environment & Perks

    + Based in Fairfield County, with flexible scheduling including occasional extended hours (8 AM–8 PM).

    + Physical requirements include standing/walking for up to 8 hours and lifting up to 50 lbs.

    Pay and Benefits

    The pay range for this position is $33.65 - $36.06/hr.
