Primary Care Physician (Full-time Remote, Telehealth)

We’re looking for a Board Certified (or Eligible) Primary Care Physician to join our team and deliver high-quality, preventive-focused care through telehealth.

What You’ll Do

+ Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)

+ Create and manage personalized care plans for ongoing conditions

+ Collaborate with our integrated care team (nurse case managers, NPs, CHWs)

+ Ensure thorough documentation in our EHR and compliance with quality measures

+ Engage patients in preventive care to reduce avoidable complications

What We’re Looking For

+ MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics

+ Active medical license — multi-state licensure a plus

+ Credentialed with major plans: UHC, Aetna, Humana

+ Strong interest in preventive care, senior health, or chronic condition management

+ Tech-savvy and comfortable practicing via telehealth

+ Collaborative, mission-driven mindset with excellent communication skills
