Pennsylvania Hospital is seeking a Clinical Dietician Specialist to join our Cancer Program team. This is a full time, day shift opportunity. The ideal candidate has experience in oncology.
Summary:
+ Responsible for the nutritional care of patients at Pennsylvania Hospital. Functions as an advanced practice, professional consultant to the health care team providing high quality evidence-based nutrition care for varied patient populations in the in-patient. Practices clinically at an advanced level due to past experiences that allow for recognition of patterns that guide clinical practice. Offers expertise in the nutrition management of disease specific or specialized patient populations including the care of patients receiving enteral and parenteral nutrition. Performs complex nutrition assessments and provides nutrition education to patients in various settings (in-patient, out-patient, support groups, community). Develops specialized nutrition care plans.
+ Represents Clinical Nutrition in multidisciplinary patient care rounds or committees for the patient populations served. Has developed leadership skills and is a role model and positive influence for others. Actively participates in the development and implementation of departmental education programs (continuing education, education of students and professional visitors) and quality assurance and performance improvement initiatives and outcomes projects. Evaluates, interprets and applies evidence-based practice to the delivery of nutrition care. Participates in the development of evidenced–based policies/procedure and practice guidelines. Presents lectures/in-services within UPHS and for outside professional organizations. The Clinical Dietitian Specialist demonstrates clinical leadership by participation in committees and professional organizations. The Clinical Dietitian Specialist recognizes the need for practice change and assists leadership in evidence-based process changes and the dissemination of information and ongoing evaluation of a practice change. Provides nutrition care and practices as part of an interprofessional team within the Clinical Nutrition division of the Food and Nutrition department.
Responsibilities:
+ Performs and documents comprehensive, accurate and timely nutrition assessments as specified by departmental policies and practice guidelines to create a nutrition care plan based on the individual patient assessment. Completes timely reassessments and reevaluation of the nutrition care plan in response to patient’s outcome. Maintains patient confidentiality.
+ Provides nutrition education to patient/families taking into consideration patient age, cultural and psychosocial needs in multiple settings. Identities barriers to learning and adapts education approach as needed.
+ Collaborates with multidisplinary team through patient care rounds and communicates with the healthcare team on a regular basis to implement complex nutrition care plans.
+ Consistently cares for specialized patient populations with complex disease specific needs and/or those receiving specialize nutrition support therapy.
+ Demonstrates skills and knowledge necessary to assure that nutrition assessment, treatment and care is appropriate based on the unique issues of the neonate, child, adolescent, young adult, or older adult patient. Assess and interprets any age-specific findings or education needs and documents these in the nutrition care plan.
+ Consistently meets departmental standards for productivity. Participates in department quality improvement initiatives and hospital committees. Provides clinical leadership within the department and as a department representative throughout the hospital.
+ Assumes responsibility for maintaining knowledge of department and hospital processes and information. Assumes responsibility for continuing education and professional development and provides documentation as needed. Actively participates in development of education materials for patients and other care team members.
+ Participates in entity and department wide initiatives for patient/employee safety. Is aware of patient/employee safety while carrying out daily tasks. Keeps up to date with all annual competencies required by UPHS/PAH.
Credentials:
+ Registered by the Commission on Dietetic Registration as a Registered Dietitian (RD)/Registered Dietitian Nutritionist (RDN) (Required)
+ State of Pennsylvania Licensed Dietitian Nutritionist (LDN) within 6 months of hire (Required)
Education or Equivalent Experience:
+ Bachelor of Arts or Science (Required)
+ Completion of an Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited didactic program in dietetics (Required)
+ At least 3 years' experience as an RD at PAH/Penn Medicine or >5 years of clinical experience as an RD outside Penn Medicine (Required)
+ Accredited Dietetic Internship Program, Individualized Supervised Practice Pathway(ISPP) or integrated supervised practice training to meet accreditation standards. (Required)
+ Any of the following specialty certifications; Certified Nutrition Support Clinician (CNSC), Board Certified Specialist in Obesity and Weight Management (CSOWM), Certified Diabetes Educator (CDE), Board Certified Specialist in Oncology (CSO), Board Certified Specialist in Renal Nutrition (CSR), Board Certified Specialist in Gerontology (CSG), Advanced Practice Certification in Clinical Nutrition (RD-AP, RDN-AP)(Required)
+ Master of Arts or Science (Required)Education Specialization: EFFECTIVE only if initial date of registration (RD, or RDN) is after 1/1/2024
