Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not a cure, and quality of life, not duration. As a Home Health Aide on our Hospice team providing coverage over Tuscarawas County, you will care for patients where they need you most, whether at home, a nursing facility, assisted living center or other location. In this role, you can help enhance their outlook on their experience, even when their futures seem uncertain. _Alongside Tuscarawas County, the caregiver in this role may need to travel to Harrison, Holmes, Stark, Coshocton and Carroll counties._
This is a part-time position. A caregiver in this role will work Monday through Friday, 2-3 days per week, between 8:00am to 5:00pm. No weekend or holiday requirements.
A caregiver who excels in this role will:
+ Perform basic patient care activities.
+ Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate.
+ Assist patients with tending to personal care, activities of daily living and transfers/transport.
+ Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.
+ Assist with keeping the patient's environment clean and orderly.
+ Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
+ Escalate change in patient condition to patient's physician as appropriate.
Minimum qualifications for the ideal future caregiver include:
+ High School Diploma or GED
+ State Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse's Aide Registry
+ One year of clinical experience as an Aide in an acute, rehab or skilled nursing setting
+ Valid Driver's License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage
+ Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross within orientation period
Preferred qualifications for the ideal future caregiver include:
+ Hospice and Palliative Care Nurse Assistant (CHPNA) Certification
+ Home Care experience

Physical Requirements:
+ Must be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient.
+ For field staff only: Able to drive to patients' homes and/or facilities.
+ Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
+ Physical Demand requirements are in excess of those for Light Work.
Personal Protective Equipment:
+ Follows standard precautions using personal protective equipment as required.
