The Case Manager serves as a key member of the patient care delivery team in a level one trauma designation, ST-Elevation Myocardial Infarction (STEMI) receiving center designation, and Comprehensive Stroke Center and Chest Pain Center accreditation. The role involves working efficiently in a fast-paced environment, utilizing clinical expertise, discretion, and independent judgment in performing the nursing process within a patient-centered care professional practice model.
Responsibilities
+ Maintain a solid working knowledge of specialized case and utilization management methodologies and practices.
+ Ensure medical appropriateness criteria are met for the level of care provided and documented from admission through discharge.
+ Collaborate in a healthcare environment to oversee the implementation of an interdisciplinary plan of care with an individualized discharge plan.
+ Reevaluate the plan of care and ensure continued appropriateness based on the patient’s changing needs and condition.
+ Exhibit a positive mindset with changes in the workforce, clinical practice, and operational setting.
+ Engage in opportunities to influence decision-making for bedside clinical practice.
+ Work calmly and respond courteously when under pressure.
Essential Skills
+ Minimum 2 years of acute inpatient experience.
+ Experience in trauma level 1 hospital and discharge planning.
+ RN license with at least 2 years of acute care case management experience.
+ Active and unencumbered California RN license.
+ Active BLS provided by the American Heart Association.
+ Bachelor’s degree in nursing.
+ Minimum of two years inpatient acute care experience.
+ Minimum of one year of Hospital Case Management or Hospital Utilization Management experience.
Additional Skills & Qualifications
+ Nationally recognized certification in Utilization Management or Case Management preferred.
+ Strong knowledge of case and utilization management practice and methodologies.
+ Familiarity with state and federal healthcare regulations and accreditation.
Work Environment
This role offers the opportunity to work at a level one trauma facility within a Magnet Recognized large tertiary academic medical center. The organization emphasizes continual learning and development, providing substantial career-pathing within a large case management team. The work involves shifts in a 400-bed trauma level 1 acute setting, requiring availability to work every other weekend.

This is a Contract position based out of Loma Linda, California.
Pay and Benefits
The pay range for this position is $50.00 - $70.00/hr.
