Salary 70K-90K
Offer Based on Years of Experience
Key Responsibilities:
+ Attend staff meetings, educational programs, and community events at UCHealth to stay informed and network with key stakeholders.
+ Communicate patient needs and potential solutions to AccentCare supervisor as well as rounding physician, social worker and charge nurse at UCHealth hospital.
+ Explain hospice services and benefits to patients and their families in a clear and concise manner as well as discussion levels of care.
+ Focus is on GIP admissions as well as community referrals.
+ Obtain signatures on required forms and provide copies to patients/responsible parties.
+ Maintain professional relationships with healthcare professionals and organizations.
+ Weekly meeting with AccentCare national leadership on growth of partnership with GIP
+ Biweekly meeting with UCHealth key stakeholders to review referrals and discuss challenges
+ Quarterly presentation to UCHealth stakeholders related to successes and programs offered through AccentCare – PowerPoint
+ Tracking GIP conversion and discussion of opportunities for increasing referrals, conversions, and community referrals.
+ Work with hospitals and facilities to identify and transition patients appropriately for general inpatient services outside of UCHealth to ensure warm hand off
+ Train new team members on GIP and orient to UCHealth hospital healthcare systems with introduction
+ Track EMR and access to Epic, Haiku and charting audits to ensure documentation is detailed and warrants GIP LOC and is downloaded in HCHB

Qualifications
Be the Best Clinical Liaison RN You Can Be
If you meet these qualifications, we want to meet you!
+ One (1) year of RN experience in local market with a strong knowledge of governmental home health regulations, Medicare requirements, and care plan development techniques
+ Previous medical sales experience preferred
Required Certifications and Licensures:
+ Licensed RN currently licensed as a registered nurse in the state of agency operation
+ Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order
