    **Schedule:** Full Time Days

    **Shift** : M-F, Hybrid schedule, 8:30am-5pm, no holidays, no weekends. Mon-Thursday in-person.

    The community you'll be caring for: AdventHealth Orlando

    **The role you'll contribute:** The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies, and/or require assistance with transitions of care or discharge planning. In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, RN Care Managers, staff nurses, and other members of the care team).

    The expertise and experiences you'll need to succeed** **:

    **Minimum qualifications** :

     

    Current degree as a Master-level (MSW) Social Worker in applicable state

    Preferred qualifications:

    + **Clinical Knowledge & Experience**

    + Background in maternal-fetal medicine (MFM), OB/GYN, or related high-risk perinatal care.

    + Familiarity with maternal specialty needs (cardiology, endocrinology, nephrology, hematology, rheumatology).

    + Understanding of fetal specialty care pathways (cardiology, neurology, pediatric surgery, genetics, NICU, etc.).

    + **Care Coordination & Multidisciplinary Collaboration**

    + Experience coordinating complex care across multiple specialties (OB, MFM, NICU, palliative, pediatric subspecialists).

    + Skilled at organizing and facilitating multidisciplinary meetings, ensuring follow-up on tasks and communication.

    + Ability to provide warm hand-offs and support transitions of care (antenatal NICU postpartum).

    + **Patient & Family Support**

    + Strong background in emotional support, counseling, or social work within maternal or pediatric health settings.

    + Experience educating families about palliative care roles and facilitating NICU/L&D tours.

    + Ability to provide timely follow-up for families experiencing high-risk pregnancies or adverse fetal diagnoses.

    + **Operational & Communication Skills**

    + Proficiency in using EMR systems (e.g., EPIC), workqueues, and team messaging for referrals and documentation.

    + Strong organizational skills for scheduling specialist appointments, imaging follow-ups, and interpreter services.

    + Effective communication skills to liaise between providers, patients, and support staff.

    + **Program Development & Continuous Monitoring**

    + Experience tracking census data, monitoring patient flow, and supporting program outcomes.

    + Ability to identify referral sources, coordinate intake processes, and streamline workflows.

    + Current, active registration with the applicable State Department of Health as a Licensed Clinical Social Worker (LCSW)
