Licensed Practical Nurse – Home Health
**Location:** Vancouver, WA
**Schedule:** Full Time, 1.0 FTE, Day shift
**Salary Range:** $28.36 – $42.03 per hour (dependent on education, training, experience, and collective bargaining agreement terms)

We are currently seeking a **Licensed Practical Nurse (LPN) – Home Health** to join our Vancouver team. This role is an excellent opportunity for an LPN who values teamwork, professional growth, and making a meaningful difference in the lives of patients and families.
What you’ll do:
+ Provide direct nursing care under the supervision of an RN, following physician orders and established care plans.
+ Administer prescribed medications and treatments, and monitor patient responses.
+ Document all care accurately in accordance with department standards.
+ Assist in the development and evaluation of patient-specific care plans.
+ Perform wound care, dressing changes, vital sign monitoring, patient education, and other direct care duties.
+ Collaborate with the healthcare team to ensure safe, timely, and patient-centered care.
+ Support daily operations by transporting supplies, equipment, and specimens as needed.
+ Maintain professional effectiveness through ongoing education and development.
Qualifications:
+ Graduate of an accredited State Board of Nursing LPN program.
+ Current Washington State LPN license.
+ Current Basic Life Support (BLS) certification.
+ Valid driver’s license, auto insurance, and reliable transportation.
+ Minimum 1 year of LPN clinical experience preferred.
+ Knowledge of age-specific principles of growth, development, and life stages.
