National Youth Advocate Program is looking for an Intake Coordinator for their Matteson, IL. location.
Position Summary
The Intake Coordinator is responsible for helping in recommending appropriate services and completing assessments for new youth entering foster care. In addition, they will assist with assessments and referrals for other programs as needed.

RESPONSIBILITIES
The Intake Coordinator will perform duties including, but not limited to:
+ Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies.
+ Be responsible for the placement procedures, interagency correspondence, and referral information for files.
+ Maintain health records in both physical and electronic formats
+ Process referrals and complete enrollments for OPBH, MHJJ (Matteson & Rockford), and the Psychiatry Clinic, ensuring all documentation is accurate and up to date.
+ Process client discharges for OPBH and MHJJ programs at the Matteson and Rockford offices, ensuring proper documentation and timely closure of services.
+ Manage incoming online referrals across Chicago, Matteson, and Rockford locations, ensuring proper routing and follow-up
+ Verify Medicaid coverage during referrals and enrollments to ensure client eligibility and respond to staff requests for one-off insurance checks for potential clients.
+ Serve as the lead point of contact for the psychiatry clinic, ensuring smooth communication and coordination between providers, caregivers, and agency staff.
+ Manage and maintain the clinic schedule, proactively addressing scheduling conflicts and ensuring timely appointments for youth in care.
+ Communicate with foster parents, caregivers, and the DCFS medical consent line regarding medication management, questions, and issues related to submitted consents.
+ Be responsible for the intake assessments for OPMH.
+ Recommend appropriate services such as psychiatry or MCR services.
+ Communicate with referring sites regarding availability of care needed.
+ Assist with increasing the referral base by approved methods and strategies to keep child-placing agencies aware of NYAP’s continuum of care and services. Such as working with Program Excellence to develop strategies for outreach to the community through ads.
+ Attend community events to keep other agencies aware of NYAP’s program offerings.
+ Coordinate with internal NYAP foster care staff to confirm that all youth receive appropriate and timely medical and mental health appointments ..
+ Act as the primary point of contact for medication refill requests, relaying information to the prescribing doctor and ensuring timely fulfillment.
+ Engage clients and caregivers through follow-ups to confirm appointment attendance, provide service updates, and address questions or concerns.
+ Facilitate clear, consistent communication between clients, caregivers, providers, and staff to support effective care coordination.
+ Arrange conference/training for OPMH Team
+ Attend meetings and distribute meeting minutes
+ Assist in front desk coverage, 1-2 days per week
+ Interact effectively with internal and external stakeholders.
+ Performs other duties as requested.
Minimum Qualifications:
+ A Bachelor’s degree is required. Master’s Degree preferred in social services or human service-related field.
+ Minimum two years’ experience in youth services, with direct experience in child welfare.
+ Proficient us of desktop and laptop computers, internet search, people management programs, smartphones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
+ Candidate must obtain required car insurance bodily injury 100,000/300,000
