    Office Location: Brooklyn, NY

     

    Hours: 9:00am- 5:00pm

     

    Shift: Monday - Friday

     

    Salary: $27.00/ HR + Bonus

    What You Need to Know

     

    Client Care Liaison Responsibilities

     

    + Responsible for initiating contact with new clients upon notification from the Intake Department

    + Answer and triage telephone calls coming into the staffing department

    + Monitor the EMR system and complete daily attendance verification

    + Prepare, monitor, and distribute open case/client coverage report for the Client Service Staffers and Supervisors

    + Communicate scheduling and/or service changes to clients, payors and caregivers as needed

    Qualifications

    Client Care Liaison Qualifications

     

    + Previous home health care or home care experience highly desirable

    + High School or GED required

    + Bi-Lingual English/Spanish speaking required
