Pennsylvania Hospital is seeking a full-time Clinical Pharmacy Specialist, Anticoagulation. This position is responsible for provision of comprehensive medication management to patient populations across the institution including ambulatory and acute care. This pharmacist serves as a resource to Pennsylvania Hospital on best practices, evidence-based guidelines, emerging therapies, transitions of care, optimal pharmacologic management, effective medication reconciliation, and other pharmaceutical services. This pharmacist collaborates on hospital and health-system committees, workgroups and initiatives with roles spanning active membership to committee chair. This position reports to the Associate Director, Professional Practice. The successful candidate will have demonstrated contributions to pharmacy practice, leadership skills and a personal commitment to advancing the profession.
Responsibilities:
+ Participate in patient care collaborative interactions with interdisciplinary colleagues, clinical pharmacists, residents, and students. Monitor medication therapy for appropriate selection, use, dosing, dosage form, route, therapeutic duplication and drug/dietary/disease state interactions, and document pharmacist interventions. Assist in navigation of drug shortages to limit impact on patient care.
+ Ensure continuity of care through transition in care activities for patients including education and access to medication in collaboration with the ambulatory pharmacy.
+ Provide educational in-services, newsletters, online material, and other media for health professional updates on pharmacotherapy topics. Develop competency assessments in pharmacotherapy management practices through experiential and didactic training for pharmacists.
+ Serve in a preceptor/mentor role for pharmacy residents, pharmacy clerkship students, and pharmacists.
+ Participate as an appointed member of the hospital’s Pharmacy and Therapeutics Committee as well as other hospital and health system committees to support new clinical initiatives. Contribute to assessment and selection of medications for the health system formulary.
+ Collaborate with interdisciplinary teams to ensure appropriate therapeutic uses of medication. Promote formulary adherence by prescribers.
+ Develop and implement policies and procedures that support evidence-based treatment guidelines and clinical care plans for pharmacotherapy management. Ensure that existing pharmacy policies, guidelines, and educational resources pertaining to pharmacotherapy are current with the latest clinical advances and standards of practice.
+ Develop clinical decision support tools relative to pharmacotherapy management. Provide operational support and recommendations for safe and effective use of information technology for pharmacotherapy management.
+ Implement and maintain clinical pharmacy programs that support meeting Medicare Core Measures. Effectively utilize information technology to enhance pharmacotherapy management through surveillance, utilization, and outcome reporting. Contribute to departmental and organizational performance improvement by providing recommendations for improving clinical services to the pharmacy leadership team.
+ Develop and conduct target drug programs and Drug Use Evaluations (DUEs) for selected pharmacotherapeutic agents. Report findings to Pharmacy and Therapeutics Committee.
Credentials:
+ Current Registered Pharmacist license in the Commonwealth of Pennsylvania or eligible for licensure (required)
• Current Certified Anticoagulation Care Provider from the National Certification Board for Anticoagulation Providers or eligible for certification within 18 months of hire (required). If not currently certified, must be obtained within 2 years of hire
Education or Equivalent Experience:
+ Doctor of Pharmacy Degree from an accredited College of Pharmacy (required
+ Completion of a Pharmacy Practice Residency (PGY-1) or current Board of Pharmaceutical Specialties Pharmacotherapy Specialist certification (required)
+ Successful completion of a PGY-2 Thrombosis and Hemostasis Management Residency OR two years of experience as a licensed, full-time pharmacist in a hospital OR two years of experience as a licensed, full-time pharmacist in an ambulatory practice with ≥ 50% of time spent with providing anticoagulation-related patient management (required)
+ Experience precepting students and residents (preferred)
+ Personal commitment to advancing the profession
+ Demonstration of contributions to pharmacy practice and leadership skills
