    Summary Diagnostic ultrasound uses high frequency sound waves and other diagnostic techniques for medical purposes. The practitioner must be competent in the production, use, recognition, and analysis of ultrasound images and patterns used for patient diagnosis and treatment. The sonographer is regarded as the expert source of all ultrasound imaging, and the interpreting radiologist relies heavily on the skills, knowledge, and abilities of the sonographer in providing a final interpretation. Responsibilities The Medical Instrument Technologist coordinates a variety of technical procedures that require independent judgment, ingenuity, and initiative in the application of non-ionizing high frequency ultrasonic waves to humans for diagnosis of disease in the clinic or on the wards. Duties may include but are not limited to the following: The incumbent performs ultrasonography studies, records, and processesimages, as well as prepares the processed media for interpretation by the StaffRadiologist. The studies include: Abdomen-liver, gallbladder, spleen, pancreas,abdominal aorta, vena cava and femoral arteries using Gray Scale techniques toevaluate these structures for size, abnormal contour, cysts, tumors and variouspathological disorders. Retroperitoneal - kidneys, adrenal glands, nodes, andurinary bladder using Gray Scale real time techniques. Pelvis - urinary bladder,scrotum, prostate, MRI prostate fusion studies. Also performs segmentalpressures, pulse volume recordings and arterial Doppler waive forms. The employee assists with percutaneous punctures as directed by theRadiologist. The employee assists with studies localizing fluid filled areas underultrasound and employs sterile technique as indicated for punctures and biopsies. The employee performs studies for thyroid, venous and arterial vessels. Wardexams are at the discretion of the Staff Radiologist and will include: gallbladder,pancreas, and visualization of the intra-hepatic biliary system, abdominal vessels,liver, and kidney, venous and arterial examinations. The employee schedules and prepares patients for exams and explains procedures to patients while performing the studies. The employee obtains pertinent diagnostic examinations for the physician, which may be necessary for correlation between the ultrasound scan and the clinical history. The employee differentiates artifacts from normal or pathological processes and recognizes electronic equipment limitations. The employee recognizes a diagnostic scan and consults with the Staff Radiologist to determine the proper diagnosis. The employee performs and completes imaging studies within the established guidelines for stat, ER, and inpatient orders. Maintains current Basic Life Support (BLS) and professional credentials. Maintains current Talent management System (TMS) training as required. Technologist works under the supervision of the Lead Technologists and rotates all responsibilities within the US section. He or she works independently on most exams but confers with radiologists and or Lead Technologist on more difficult problems. Work is spot checked for achievement of objectives Work Schedule: Monday - Friday, 7:30 am - 4:00 pm Recruitment Incentive (Sign-on Bonus): Not Authorized Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). BASIC REQUIREMENTS Citizenship. Citizenship of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3g, Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. See the grade requirements part of this standard for information regarding educational substitutions. Licensure or Certification. Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. For certain functional areas at the higher levels or supervisory assignments, specific certifications appropriate to the specialty are indicated in this standard under "Titles and Certification/Registrations". For all specialties identified in this standard, Basic and Advanced Cardiac Life Support certifications are desirable. Training which does not result in official certification does not meet this definition.d. Physical Requirements. See VA Directive and Handbook 5019. English Language Proficiency. Medical Instrument Technicians must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, Chapter 2, Section D, Paragraph 5a. GRADE DETERMINATIONS Creditable Experience. Knowledge of Current Medical Instrument Technician Practices. To be creditable, the experience must have required the use of knowledge, skills, abilities and other characteristics associated with current Medical Instrument Technician practice appropriate to the identified specialty area. Quality of Experience. Experience is only creditable if it is equivalent to at least the next lower grade level and is directly related to the position/specialty to be filled. Part-time Experience. Part time experience is credited according to its relationship to a full time work week. For example, an individual employed 20 hours per week, or on a ½ time basis would receive one work week credit for each two weeks of service. (This must be included in your resume) Medical Instrument Technician (Diagnostic Ultrasound) - GS-09 (Above Full Performance Level) - In addition to the core competencies defined at the GS-8 full performance level, positions at this grade may be lead, supervisory, or non-supervisory with specialized advanced knowledge and assignments. Experience. At least 1 year of experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at the GS-08 level. This would be specialized experience which demonstrates possession of the knowledge required in order to provide all aspects of diagnostic ultrasonography services. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*): Demonstrated Knowledge, Skills, and Abilities. KSAs. In addition to the GS-09 level Experience, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*). 1, 2, and 3 are appropriate for all positions at this level. KSAs 4 and 5 are to be used in accordance with their appropriateness for the specific assignment. Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols that pertain to diagnostic ultrasonography. * Knowledge of JCAHO and other regulatory requirements. Ability to communicate effectively and professionally with employees at varying grade levels. * Ability to manage and supervise employees. * Ability to provide, or provide for, staff development and training. Certification. Registration by Cardiovascular Credentialing International (CCI) or by the American Registry of Diagnostic Medical Sonographers (ARDMS) is highly desirable. Basic Cardiac Life Support (BCLS) certifications are also desirable Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time. Preferred Experience: Experience with GE/Siemens/Vascular is preferred.
