    Position will cover Suffolk County

     

    + 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

    + Great Healthcare benefits, and education reimbursement of $7,000 annually

    + Additional compensation for visits over your caseload

    Job Details

    Our Home Care Behavioral Health RN’s:

    + Develop a specific behavior health plan of care based upon comprehensive assessment, patient input and consultation with physician and incorporate patient’s language, cultural and psychosocial needs into the care plan

    + Coordinates referrals for appropriate services/disciplines/DME/Supplies based upon skilled nursing assessment and physician’s orders

    + Assures timely and appropriate reassessment or discharge as per agency requirements

    + Actively participates in the REACT medication reconciliation program .

    + Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.

    + Provide care to the most upmost standard

    Minimum Qualifications:

    + New York State RN license

    + Minimum of one year of recent hands on experience as a Registered Nurse.

    + Basic Life Support Certification

    + Valid New York State Driver’s License and reliable transportation

     

    Salary Range

     

    USD $100,000.00 - USD $130,000.00 /Yr.

     

    This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
