FT Registered Nurse Care Coordinator position for SLH

Requirements:

+ BSN Degree

+ BLS

+ Current MO RN license

+ Minimum of 1-yr inpatient nursing experience

- Responsible for ensuring quality patient outcomes and overcoming obstacles to delivery of care through interdisciplinary coordination and communication.

-This position coordinates the interdisciplinary team and partners with physicians to ensure patient treatment plan progress.

-The Care Coordinator will ensure that an appropriate plan of care is initiated and will monitor care progression to ensure quality outcomes and effective transitions of care

Job Requirements

Applicable Experience:

1 year

Registered Nurse (MO) - National Council of State Boards of Nursing (NURSYS)

Bachelor's Degree - Science in Nursing
