    Job Summary:

    The Transitional Care Clinical Quality Improvement Coordinator leads efforts to adhere to evidence-based care for patients as they move through the care continuum. Responsibilities include coordination and management of projects that improve quality and patient safety outcomes, analyzing data and facilitating the design, development, and implementation of process improvement initiatives based on the findings. This RN performs medical record reviews, concurrent data abstraction from the chart and other primary and secondary data sets on clinical indicators for assigned patient groups/performance improvement projects, and oversees assigned departments' incident/event reports. This RN focuses on readmission data, identifies trends, and reports findings to the Transitional Care Committee. This RN works closely with Transitional Care leadership to identify opportunities for care delivery and discharge planning improvement and assists with program development for identified gaps with the continuum in mind. Will facilitate communication and collaboration among various healthcare professionals, post-acute partners, and other relevant parties to ensure a coordinated approach to care. Must demonstrate strong organizational skills and ability to self-direct tasks to achieve goals, strong verbal and written skills to interact with a variety of professionals. A strong candidate will demonstrate commitment to improving the well-being of individuals as they move through the care continuum.

    Minimum Qualifications:

    NYS licensed RN or eligible, Bachelor's degree in Nursing or a health care related field, and three years of acute care RN experience. Excellent clinical, verbal, and written communication, customer service, and computer skills required.

    Preferred Qualifications:

    Experience working in case management preferred. Experience in data management and quality management preferred.

    Monday-Friday, Days
