Responsibilities:
GENERAL PURPOSE
This job consists of patient coordination of all new requests for services which can encompass up to 100 requests per week. The job involves extensive tracking via spreadsheets to ensure no patients fall through the cracks. It also involves communication via phone, MyChart, letters and inbasket messages with patients, families, and referring clinicians. The job requires a high level of competency and independent critical thinking.
ESSENTIAL FUNCTIONS
Patient Intake and Service Coordination:
+ Organize and track new patient requests, maintain clinical information, and provide updates to the Clinical Lead and Director.
+ Conduct intake phone screens, document in eRecord, and schedule new patient visits as needed.
+ Attend Intake Team meetings, weekly supervision, and provide coverage for team members when necessary.
+ Handle patient complaints and inquiries, ensuring prompt and courteous communication by telephone or by letter.
+ Serve as a point of contact for intake related questions and problem resolution.
Communications Coordination for Intake Team:
+ Composes, customizes and sends correspondence to families seeking care within our services (e.g., waitlist and referral correspondence; requests for clinical summaries for psychiatric consultation)
+ Communicate with clinicians, primary care providers, and school personnel via eRecord, email, and phone.
+ Ensure patients and families receive timely and professional communication.
+ Collaborate with referring providers and specialty pediatric services as needed.
+ Maintain accurate and professional correspondence records for intake-related matters.
Liaison Responsibilities:
+ Interfaces with our Brighter Days Pediatric Mental Health Urgent Care on patients needing access to crisis services and patient transfers.
+ Coordinates with Financial Counselors regarding fees and insurance issues
+ Establishes and regularly maintains cooperative working relationships with community and health care agencies and professional and administrative staff of the Medical Center and University.
+ Handles inquiries from other UR Medicine clinical services (e.g. Inpatient psychiatry, pediatric partial hospitalization in psychiatry, and pediatric clinics) as well as community providers (e.g. PCPs, school district staff) when the Clinical Intake Coordinator is unavailable.
+ Interfaces with clinical teams associated with research projects being conducted at the service.
Additional Professional Responsibilities:
+ Participate in training, workshops, and diversity initiatives to support professional growth.
+ Participate in departmental and programmatic diversity presentations and meet annual training expectations
+ Model honesty, integrity, and commitment to the mission of the Department of Psychiatry and UR Medicine.
+ Maintain knowledge of departmental policies and procedures.
+ Support the vision and mission of the Child & Adolescent Psychiatry Division.
MINIMUM EDUCATION & EXPERIENCE
+ Bachelor’s degree and one year of experience or an equivalent combination of education and experience Required
+ Proficient computer experience (with programs such as Excel) Required
