    We are hiring a Nurse Care Coordinator!

    Schedule

     

    + Full-Time

    + Monday-Friday flexible onsite during typical business hours, on call between 6 am-4 pm.

    + Rotating backup on-call schedule every 5–6 weekends, along with one holiday per year.

    How You’ll Make an Impact

     

    + Oversee healthcare needs of 16–20 residents in our group homes, ensuring high-quality care in a supportive home environment.

    + Coordinate and attend medical appointments, specialty visits, and annual physicals; complete quarterly and PRN health assessments.

    + Provide phone support to direct support professionals, medication managers, and CMAs from 6 AM–4 PM; conduct weekly (and as-needed) visits to each group home.

    + Maintain medical charts in compliance with state regulations; deliver staff training in areas such as G-tube care, seizure management, and medication administration.

     

    What You’ll Need

     

    + Graduate of an accredited School of Nursing and licensed to practice as a Registered Nurse in the State of Iowa. LPN will be considered if pursing an RN degree.

    + Nursing experience in the field of developmental disabilities, pediatrics, and behavioral health preferred.

    + Must have reliable transportation and be willing to become an authorized driver with Child Serve and use own car to transport children in the community.

    + Ability to lift up to 35 lbs, be mobile within the clinic areas, and frequently bend, lift, reach, and stand.

    + Active BLS from the American Heart Association or American Red Cross

    + Ability to understand and communicate effectively in (or through) reading, writing, and speaking the English language.
