Job Summary and Qualifications
Primary Purpose of the job Assesses patients and their families for educational needs relevant to wound care. Assesses patients for physiological, psychological, comfort and safety needs. Serves as a clinical resource for the nursing staff and health care providers to assist with planning strategy of care either pre- or postoperatively or ongoing. Promotes optimum independence and return to self-care as applicable to each patient’s individual circumstances. Responsibilities include direct nursing intervention provided to a variety of age populations and diagnostic categories, based on the physician developed plan of care and established priorities for wound care issues.
What you will do in this role:
+ Reviews medication and provides patient education, pain medication administration per physician order, accurately assess the pain levels and effects of administered pain medications, upholding professional nursing license and complete traditional nursing duties and expectations.
+ Identifies potential cause(s) of delayed wound healing; such as vascular disease, infection, diabetes, or other metabolic disorders, immunosuppression, unrelieved pressure, radiation injury, tobacco use, patient noncompliance and malnutrition and modifying treatment as directed by the physician as the wound progresses through the stages of healing; provides provision of specialized wound care to include but not limited to debridement sharp, autolytic, enzymatic and mechanical pulsed lavage, whirlpool, wet to dry dressings, and high pressure water.
+ Manages edema, application of antimicrobial agents, application and management of wound vac, application and removal of dressings of all kinds; fits and measures for compression stockings and/or garments both pre- made and custom.
+ Orders needed supplies to carry out patient care to include wound vac supplies. Inputs orders to process wound culture. Obtains correct pharmaceutical agents/solutions per physician order. Completes wound care documentation forms, written and via Meditech. Handles, processes and manages instruments including tweezers, scissors, scalpels, and other instruments as patient care requires.
+ Reviews medical record to obtain patient history, progress, and current orders. Assesses patient readiness to learn self-care. Instructs patients and families using a variety of methods as appropriate.
+ Documents actions taken along with patient response in the medical record. Provides for discharge planning assistance by communicating and collaborating with appropriate healthcare providers regarding wound and skin care. Provides for continuity of care by telephone, personal visit, or written information to community healthcare agencies.
+ Assists with the development of departmental policies and procedures to ensure compliance with hospital, state, federal, and Joint Commission guidelines. Assists with the development of and management of the annual departmental budget to ensure that it meets departmental and hospital goals for successful and cost- effective operations. Develops policies and procedures and standards of care for skin and wound care management.
+ Reviews literature for current information and obtains current educational resources for patient and staff utilization. Participates in orientation program for new nursing staff. Serves as an educational resource for area healthcare facilities, clinicians, and organizations (such as the American Cancer Society) Assists in development of policies and procedures and standards of care for stoma, skin, wound care and incontinence management.
+ Promotes quality of care by identifying, planning, and implementing educational opportunities in conjunction with the department supervisor. Assists staff and families as a liaison between physicians and other disciplines within and outside of the hospital. Interfaces closely with employees and representatives from outside the facility to ensure positive working relationships.
+ Submits daily billing sheets, follows through on assignments and anticipates and prepares for forthcoming issues and projects, supports other programs as appropriate, utilizes unit physical resources appropriately, and participates in preventive care to prolong the work life of unit equipment.
+ Operates bladder scan ultrasound equipment and safety equipment to include gait belts, safety belt, grab bars, adaptive equipment, stretchers and beds. Uses debridement tools, antimicrobial agents, wound vacs, and dressings of all kinds. Uses mobility equipment to include walkers, canes, and wheelchairs. Uses wound equipment to include use of sharps, whirlpool, etc.
What qualifications you will need:
+ Current Idaho RN License. Compact license accepted but must obtain Idaho RN license within 90 days of date of hire
+ Associate degree in Nursing required. Bachelor's degree in Nursing preferred.
+ Wound care experience required. Ostomy experience preferred.
+ Basic Cardiac Life Support must be obtained within 7 days of employment start date.
+ Current CWOCN or CWCN certification preferred or obtained within one year of hire.
