Are you a detail-oriented, human services professional who excels in a fast-paced environment? Do you want to make a positive impact on the lives of Idaho's children and families? TheService Coordinatorposition provides service coordination and case management to families with children birth to three with developmental disabilities. The service coordinator will work with families as well as a multidisciplinary team to determine and implement services in the child's natural environment. This position will be located out of theIdaho Falls office.
The Idaho Department of Health & Welfare is a drug-free workplace. EXAMPLE OF DUTIES:Provide Service Coordination to families with children experiencing developmental delays, or those who have medical or other conditions that may lead to a developmental delay.
+ Facilitate, coordinate and participate in the development, review, and implementation of Individualized Family Service Plans.
+ Meet with families routinely to evaluate progress on outcomes, review priorities, and identify service needs and options.
+ Provide information to parents and primary caregivers on resources and supports available to them.
+ Facilitate the development of a transition plan to preschool and/or other community option with the family and if applicable, schedule and facilitate the Transition. Planning Meeting with family and School District.
+ Complete accurate documentation and data entry in accordance with policies, procedures and within established timelines.
+ Participate in child find and community outreach activities.
MINIMUM QUALIFICATIONS:
You must possess all the minimum qualifications below to pass the exam for this position. Please make sure your resume or work history supports your meeting the minimum qualifications for this position. Failure to do this may disqualify you from being considered for this position. It is highly recommended to attach a one-page cover letter to your application to demonstrate how you meet the requirements below.
+ Good knowledge of human services principles and practices including behavioral management methods, social and family systems, cognitive behavioral education and case management methods. Typically gained by a certification as a psychosocial rehabilitation specialist in accordance with USPRA requirements OR bachelor's degree in social work, psychology, counseling, behavioral science, special education, psychosocial rehabilitation, healthcare, criminal justice or related field from a nationally accredited university or college OR 2-4 upper division courses as stated above AND 2-4 years’ work experience OR experience that represents the knowledge level above.
+ Computer literacy for data entry, electronic mail, composing reports and letters, and storing and retrieving documents.
+ Valid driver's license.
Below is preferred experience. It is not required for the position but applicants with this experience may receive consideration over other applicants.
+ Good knowledge of Infant Toddler Program, early childhood development, children's developmental disabilities, early childhood medical conditions, and family centered services.Typically gained through at least two upper division courses in special education, elementary education, early childhood education, or closely related field OR at least one-year experience working in a program that delivered early intervention services to infants and toddlers with disabilities.
+ Completion of specialized training in developmental disabilities OR one-year experience treating the developmentally disabled. Experience should include working as part of a multidisciplinary team, working with people with developmental disabilities, developing, implementing and evaluating individual treatment plans.
+ Experience performing case management review and analysis.Typically gained by one-year work experience in case management, case plan development and monitoring, and assessing client strengths and weaknesses.
+ Ability to speak, read and write in Spanish.
