    What you'll do

     

    Under the supervision by the attending physician and the Program Coordinator for Diabetes Management, the Certified Diabetes Nurse Specialist acts as an expert resource in the field of diabetes. The Certified Diabetes Nurse Specialist functions as a critical team member of the Endocrine Division, delivering care to patients in this subspecialty. Contributes to education of family, community and hospital staff as appropriate, and participates in medical and nursing research as indicated. The Certified Diabetes Nurse Specialist works closely with the Program Coordinator on such issues as quality assurance, and equipment ordering when necessary. This individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served. This individual must demonstrate the ability to assess data reflective of the patients’ status, identify each patient’s requirements relevant to his or her age specific needs, and provide appropriate care. The Certified Diabetes Nurse Specialist supports and advocates for people with diabetes to optimize quality care. The Certified Diabetes nurse specialist promotes self-management to achieve treatment goals that reduce risks and optimize health outcomes.

     

    + Provides care to patients primarily in the outpatient setting. Utilizes principles of nursing process in meeting patient age specific needs with regard to safety, hygiene, nutrition, and medication administration while also attending to physiological, spiritual and emotional needs

    + Collects patient health data and reviews with physician as appropriate

    + Conducts/coordinates outpatient diagnostic tests/treatments according to established protocols

    + Identifies expected outcomes individualized to patient

    + Provides appropriate patient and family teaching and support

    + Explains treatments and procedures and answers patient questions

    + Participates in the development of patient teaching materials

    + Advises modifications of self-care within treatment plan and refers to physicians, as necessary

    + Receives phone inquiries from patients, identifying issues and concerns and the need for appointment or appropriate intervention

    + Assists new clinic nurses on specifics of Diabetes Management

    + Act as an expert resource in the field of diabetes

    + Functions as a Diabetes resource for the Bassett Healthcare Network clinics assigned

    + Works to arrange or participate in-services on diabetes management and related topics

    + Helps with orientation schedules for new nurses and staff as assigned

    + Assists with orienting new Diabetes Nurse Specialists

    + Working closely with the Program Coordinator, functions as a key member of the Diabetes Education program and the Endocrinology Team

    + Set an example in work performance and team building attitudes, concern for patients and support of clinicians by maintaining a positive interpersonal attitude and demeanor, as observed by the program coordinator

    + Resolves patient issues as they relate to Diabetes Management operations, forwarding to the Program Coordinator as necessary, and as observed by the Program Coordinator

    + Works with Program Coordinator to assure compliance with all outside accrediting and regulatory agency codes and standards, as observed by the Program Coordinator

    + Works directly with the Program Coordinator to implement Quality Assurance/Risk Management activities and practices, as observed by the Program Coordinator

    + Assists and/or provides teaching and consultative services to nurses, other health professionals, community health providers, and participates in public education, as time permits

    + Assists with educational programs to nurses, students, and other staff members to improve knowledge of the care of diabetes patients

    + Coordinates and presents educational programs to nurses, students, and other staff members to improve knowledge of the care of diabetes patients

    + Participates on educational activities of other departments and community programs

    + Participates on committees, reports nursing aspect of patient care, as indicated

    + Provides quality diabetes self-management education in individual and group setting based on assessed needs, utilizes appropriate teaching techniques that are sensitive to the learning preference of the person with diabetes

    + Assists Program Coordinator with financial accountability including oversight of equipment ordering and maintenance processes and department billing

    + Is knowledgeable in correct billing methods, ensuring EMR notes, and billing codes are completed correctly with 90-95% accuracy rate

    + Ensures that supplies are ordered in the most cost-effective manner possible, alerting program coordinator of potential issues as they arise

    + Follows the ADCES diabetes education accreditation program requirement which are based off of national standards of diabetes self-management and support guidelines

    + Performs other duties as requested and observed by the Program Coordinator

     

    What we need from you

    Education:

    + 4 Year/Bachelor's Degree in Nursing, required

    Experience:

    + Minimum 5 years RN Nursing experience, required

    Licensure/Certifications:

    + Current NYS RN License, required

    + Certified Diabetes Educator certification, required

    + BLS certification, required

    + Must complete American Associates of Diabetes Educators 1 program within 8 months from date of hire, required

    Physical Requirements:

    + Light Work - Exerting up to 20 lbs of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg control requires exertion of forces greater than that of sedentary work and if the worker sits most of the time, the job is considered light work

    + Occasional crouching, kneeling, lifting, pulling, pushing, reaching, standing, stooping, walking

    + Frequent typing/clerical/dexterity, grasping, hearing, repetitive motion, seeing/monitor/computer use, talking, visual acuity (color, depth, perception)

    Hazards:

    + Occasional bodily fluids/bloodborne exposure

    Travel:

    + Extensive Travel - You should expect to travel every week

    Pay Transparency

     

    Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement.

    Pay Range Minimum:

    USD $89,576.05/Yr.

    Pay Range Maximum:

    USD $134,364.07/Yr.
